目的 比较Cushing溃疡与慢性消化性溃疡血清胃泌素水平的差异 ,探讨其发病机制及预防Cushing溃疡的有效方法。方法 采用放射免疫分析法 ,分别测定Cushing溃疡与慢性消化性溃疡血清胃泌素水平 ,结果用均值±标准差表示 ,统计分析采用t检验。结果 慢性溃疡组 3 7例 ,平均胃泌素水平 ( 10 3 2± 2 2 4)pg/ml;Cushing溃疡组 2 6例 ,平均胃泌素水平 ( 12 8 1± 16 2 )pg/ml。Cushing溃疡组高于慢性溃疡组 ,两组比较有显著差异 (P <0 0 0 1)。结论 Cushing溃疡血清胃泌素水平高于慢性消化性溃疡 ,可能因为胃内低pH值抑制胃泌素分泌 ,且应激时下丘脑、神经垂体的胃泌素分泌也增加 ,抑酸治疗可能有效预防Cushing溃疡发生。
Objective To compare the difference of serum gastrin level between Cushing ulcer and chronic peptic ulcer and to explore its pathogenesis and effective method to prevent Cushing ulcer. Methods Radioimmunoassay was used to determine the levels of serum gastrin in patients with Cushing ulcer and chronic peptic ulcer respectively. The results were expressed as mean ± standard deviation and t test was used for statistical analysis. RESULTS: Thirty-seven patients with chronic ulcer had a mean gastrin level of 10 3 2 ± 2 2 4 pg / ml, 26 patients with Cushing ulcer and an average gastrin level of 12 8 1 ± 16 2 pg / ml. Cushing ulcer group was higher than chronic ulcer group, there was significant difference between the two groups (P <0.01). Conclusions The serum gastrin level of Cushing ulcer is higher than that of chronic peptic ulcer. Gastrin secretion may be inhibited by low pH in the stomach and increased secretion of gastrin in the hypothalamus and neurohypophosphatemia when stressed. Inhibition of acid suppression may be effective Cushing ulcer occurs.
