目的就甲状腺素与多潘立酮治疗早产儿喂养不耐受进行疗效观察。方法选取我院自2010年2月~2013年2月所收治的51例早产低体重儿。将其随机分为治疗组、对照组,治疗组28例与对照组23例。本组所有患儿都要积极维持电解质平衡、水平衡,必要时给予对症治疗和静脉营养。患儿在出生后的6~12h之内开始喂养,每2h喂养1次,28例治疗组患儿在此基础上胃管注入或者口服3~5 mg/kg·d左旋甲状腺素和0.1~0.3 mg/kg·次多潘立酮,10~14d为一个疗程。结果 28例治疗组患儿恢复到出生体置时间为(9.50±2.40)d,住院时间为(21.30±9.70)d,残余奶量为(2.50±0.63)ml/次,胃管留置时间为(7.40±4.80)d,奶量增加为(121.00±34.00)ml/kg·d,体重开始增长时间为(7.80±3.20)d;而23例对照组恢复到出生体重时间为(12.60±2.10)d,住院时间为(26.60±10.40)d,残余奶量为(5.20±2.65)ml/次,胃管留置时间为(10.60±7.90)d,奶量增加为(81.00±22.00)ml/kg·d,体重开始增长时间为(10.20±3.90)天,二者具有较为明显的差异,具有统计学意义。结论联合口服左旋甲状腺素+多潘立酮能够提高喂养耐受性,促进早产儿胃肠动力,对早产儿胃肠功能进行有效地早期干预,使早产儿服用的奶量增加、体重增长快,效果显著,值得在基层医院大量推广使用。
Objective To observe the effect of thyroxine and domperidone on the feeding intolerance in premature infants. Methods Fifty-one preterm low birth weight children were selected from our hospital from February 2010 to February 2013. They were randomly divided into treatment group, control group, treatment group 28 cases and control group 23 cases. All children in this group should actively maintain electrolyte balance, water balance, if necessary, to give symptomatic treatment and intravenous nutrition. The increase of milk volume was (121.00 ± 34.00) ml / kg · d, and the body weight began to increase (7.80 ± 3.20) d. The weight of the control group returned to birth weight was (12.60 ± 2.10) d , Hospital stay (26.60 ± 10.40) d, residual milk volume (5.20 ± 2.65) ml / time, gastric tube indwelling time (10.60 ± 7.90) d and milk volume increase (81.00 ± 22.00) ml / kg · d , Body weight began to increase (10.20 ± 3.90) days, the two have more significant differences, with statistical significance. Conclusions Combined oral levothyroxine + domperidone can improve feeding tolerance and promote gastrointestinal motility in preterm infants. Effective early intervention in gastrointestinal function of premature infants makes preterm infants take more milk, gain weight quickly, It is worth promoting a large number of grassroots hospitals to use.
