[目的]检测月经周期正常子宫内膜层和肌层以及子宫内膜癌组织中淋巴管密度(LVD),分析其与子宫内膜癌淋巴结转移的相关性。[方法]收集正常子宫全层标本30例(15例增殖期,15例分泌期),子宫内膜癌标本40例(20例Ⅰa期,20例Ⅲc期),应用D2-40抗体进行免疫组化染色,显微镜下计数LVD。[结果]在正常月经周期的子宫内膜层和肌层均有D2-40阳性染色的淋巴管,淋巴管与螺旋小动脉伴行。正常肌层LVD显著高于内膜层(10.70±1.16/mm2vs4.98±0.84/mm2;P<0.01);癌灶内部LVD(Ⅰa期3.49±0.94/mm2,Ⅲc期2.43±1.08/mm2)较正常内膜层和肌层均有所降低,差异显著(P<0.01)。而癌灶周边LVD升高,高于正常内膜但是低于正常肌层(P<0.05)。有淋巴结转移组癌灶周边LVD(8.63±2.88/mm2)较未转移组显著增高(7.09±1.22/mm2,P=0.012)。[结论]癌灶周边淋巴管与子宫内膜癌的淋巴结转移发生有关。
[Objective] To detect the lymphatic vessel density (LVD) in normal endometrial and muscular layers and endometrial carcinoma in menstrual cycle, and to analyze its correlation with lymph node metastasis of endometrial cancer. [Methods] Thirty cases (15 cases of proliferative phase and 15 cases of secretory phase) of normal uterus were collected, and 40 cases of endometrial carcinoma (20 cases of stage Ⅰa and 20 cases of stage Ⅲc) were collected and immunized with D2-40 antibody Staining, counting LVD under the microscope. [Results] The lymphatic vessels with D2-40 positive staining in the endometrium and muscular layer of normal menstrual cycle were accompanied by the lymphatic vessels and spiral arterioles. LVD in normal myometrium was significantly higher than that in intima (10.70 ± 1.16 / mm2vs4.98 ± 0.84 / mm2; P <0.01); LVD in internal lesion (3.49 ± 0.94 / mm2 in stage Ⅰa, 2.43 ± 1.08 / mm2 in stage Ⅲc) Normal endometrium and muscle layers were reduced, the difference was significant (P <0.01). However, the LVD around the foci was higher than that of the normal endometrium but lower than that of the normal muscular layer (P <0.05). The LVD (8.63 ± 2.88 / mm2) in the lymph node metastasis group was significantly higher than that in the non-metastasis group (7.09 ± 1.22 / mm2, P = 0.012). [Conclusion] The lymphatic vessels around the foci are related to the lymph node metastasis of endometrial carcinoma.
