目的 :探讨手术治疗食管癌伴高血压的适应证和术后血压下降的原因。方法 :对 40例食管癌伴高血压的患者 ,依据术前血压升高的程度 ,进行不同的术前准备 ,术中、术后严格控制血压。 40例均实行左侧开胸食管癌切除 ,主动脉弓上食管胃端侧吻合术。结果 :40例患者均顺利通过手术 ,且血压在 3~ 15天后 ,都有程度不同的下降。结论 :食管癌伴高血压 ,轻度、中度、无症状者 ,术前无需特殊处理 ,对有症状的高血压和重度高血压患者 ,血压和症状控制后才考虑手术治疗。食管癌伴高血压患者在手术后其血压都有所下降。
Objective: To explore the indications of surgical treatment of esophageal cancer with hypertension and the causes of postoperative blood pressure drop. Methods : Forty patients with esophageal cancer and hypertension were treated with different preoperative preparations according to the preoperative increase in blood pressure, and blood pressure was strictly controlled during and after surgery. All 40 patients underwent left thoracic esophageal cancer resection and aortic arch esophagogastric end-to-side anastomosis. RESULTS: All 40 patients passed the surgery successfully, and the blood pressure decreased in different degrees after 3 to 15 days. Conclusion :Esophageal cancer with hypertension, mild, moderate and asymptomatic patients need no special treatment before surgery. Surgical treatment is considered only after symptomatic hypertension and severe hypertension, blood pressure and symptoms are controlled. Patients with esophageal cancer and hypertension have decreased blood pressure after surgery.
