目的 分析珠海2015年梅毒感染造成的健康负担和经济负担,为合理制定卫生政策提供参考.方法 利用珠海2015年梅毒报告数据、人口数据、死因监测数据等计算发病率和伤残调整寿命年(DALYs)等健康负担指标,利用门诊收费情况、专项调查数据和发病数估算直接经济负担,采用人均GDP数据和DALYs估算间接经济负担.结果 珠海市2015年梅毒报告发病数1135例,发病率为71.13/10万,健康负担为65.42 DALYs,负担强度为4.10 DALYs/10万,隐形梅毒发病数最多,占76.04%,三期梅毒引起DALYs最大,占93.68%,男女的报告发病数相差不大,比例1.09:1,但男性的健康负担DALYs要远高于女性,比例4.58:1,梅毒发病率和DALYs强度在80岁以上人群中达到最高.直接经济负担为805657元,人均费用710元;间接经济负担为4861750元,人均费用4383元.结论 珠海梅毒发病率处在相对较高水平,不仅要针对基数大的隐性梅毒采取相应措施控制发病率上升,还应侧重对三期梅毒的遏制,减少其造成的DALYs损失和沉重经济负担.“,”Objective This study aimed to estimate the disease and financial burden caused by syphilis in Zhuhai ,2015 so as to provide reference material for making reasonable health policies. Methods Data related to reported syphilis cases , demographic characteristics and cause-specific mortality in Zhuhai ,2015 were collected to calculate the indicators of disease burden such as incidence rate and disability-adjusted life years (DALYs). Out-patient charges , specific investigation data and case number of new syphilis patients were applied to estimate the direct financial burden ,GDP per capita and DALYs for indirect financial burden. Results The reported syphilis cases were 1135,incidence rate was 71.13/100000,the disease burden of syphilis was 65.42 DALYs and the burden strength was 4.10 DALYs/100000. The proportion of latent syphilis in all the syphilis cases was 76.04%. Tertiary syphilis occupied the highest proportion of DALYs (93.68%). The reported syphilis cases of male was similar to female(1.09:1),but the disease burden of male was higher than female (4.58:1). The incidence rate and DALYs strength of syphilis were highest in the age over 80 years. Direct financial burden was 805657 yuan,and per capita cost was 710 yuan;indirect financial burden was 4861750 yuan,and per capita cost was 4383 yuan. Conclusion Incidence rate of syphilis was in relatively high level in Zhuhai. Proper measures to control incidence rate should not be only focus on latent syphilis but also tertiary syphilis which was terribly decrease DALYs and increase financial burden.
