目的研究原发性肝细胞癌(PHcc)螺旋CT血供分型与血管内皮生长因子(VEGF)表达水平的相关性。方法对45例手术病理证实的PHcc病灶术前行螺旋CT双期扫描观察血供分型,用免疫组化SP法检测PHcc高、中、低分化者的VEGF表达水平,将两者进行对比分析。结果肝动脉供血型中VEGF表达强阳性者和阳性者均为最高,其次为肝动脉和门静脉双重供血型者。门静脉供血型和少血供型VEGF表达水平最低(P<0.001)。随着组织学分级(Edmondson-Steiner分级)的增加,VEGF表达水平增高(P<0.05)。结论螺旋CT双期扫描能够反映PHcc血供分型和血管生成情况,并能推测VEGF表达水平和组织学分级,从而有利于包括抗血管生成治疗肿瘤在内的治疗方案的选择及预后评估。
Objective To investigate the relationship between the blood supply of primary hepatocellular carcinoma (PHcc) and the expression of vascular endothelial growth factor (VEGF). Methods Forty-five cases of PHcc lesion confirmed by surgery and pathology were preoperatively examined by spiral CT double-staged scanning. The expression of VEGF in high, moderate, and poor PHCC was detected by immunohistochemical SP method, and the two were compared . Results The positive and positive VEGF expression in hepatic artery was the highest, followed by the double donor hepatic artery and portal vein. There was the lowest level of VEGF expression in portal vein blood donors and blood donors (P <0.001). With the increase of histological grade (Edmondson-Steiner grade), VEGF expression increased (P <0.05). Conclusions Spiral CT double-phase scanning can reflect the blood donation and angiogenesis of PHCC, and can predict the expression of VEGF and histological grade, so as to facilitate the selection and prognosis evaluation of the therapy including anti-angiogenesis therapy.
