目的 :应用斑点追踪成像技术研究正常人及肥厚型心肌病患者的左心室长轴纵向、短轴径向及圆周方向达峰值应变的时间,探讨肥厚型心肌病患者左心室收缩的同步性。方法:获取正常对照组(30例)、非对称型肥厚型心肌病组(35例)标准心尖四腔、三腔、两腔和短轴二尖瓣、乳头肌、心尖水平图像;分别测量和比较肥厚型心肌病组与正常对照组各节段的纵向、径向和圆周方向的收缩期达峰值应变的时间,并将所有时间与心率标化,得到标化的达峰时间。结果:正常组左室长轴纵向、短轴径向、圆周方向应变的标化达峰时间之间差异无统计学意义;与对照组比较,肥厚型心肌病组各节段纵向应变的标化达峰时间(Tl)在多数节段显著降低(P<0.05),不仅发生在心肌肥厚的室间隔,心肌厚度正常的节段也降低;短轴径向应变的标化达峰时间(Tr)、圆周应变的标化达峰时间(Tc)在部分节段也延迟,但纵向应变的标化达峰时间延迟更加明显。结论 :斑点追踪成像技术能够较好评价肥厚型心肌病患者左室整体和局部的收缩同步性。
OBJECTIVE: To study the synchrony of left ventricular systole in hypertrophic cardiomyopathy patients with hypertrophic cardiomyopathy by means of speckle tracking imaging in patients with hypertrophic cardiomyopathy and long-axis and long-axis radial and circumferential directions of left ventricle. Methods: The images of normal apical four-chamber, three-chamber, two-chamber and short-axis mitral valve, papillary muscle and apical level were obtained from normal control group (n = 30) and asymmetric hypertrophic cardiomyopathy group (n = 35) Hypertrophic cardiomyopathy group and normal control group of each segment longitudinal, radial and circumferential systolic peak strain time, and all the time and heart rate, the standard peak time. Results: There was no significant difference between the normalized group and the control group in the longitudinal and axial shortening of the longitudinal axis of the longitudinal axis of the left ventricle. Conclusions: Speckle tracking imaging can better assess global and regional systolic synchrony in patients with hypertrophic cardiomyopathy.
