本文应用超声心动图对60例慢阻肺合并肺心病患者的左心功能作了初步观察。结果提示慢阻肺肺心病患者即使在缓解期静息时SVI,CI和EF已较列照组显著降低(P<0.001),当合并有心力衰竭和呼吸衰竭时肺心病患者的心输出量减少更明显。同时观察到肺心组患者的EDV和MVCF较对照组降低,提示左室前负荷减少,心肌收缩性减弱可能是引起肺心病人心输出量减少的主要原因。此外,在4例接受机械正压呼吸的急性呼吸衰竭患者中观察到,随着机械呼吸通气量的增加,每搏输出量则逐渐减少,超声心动图能有助于我们正确估计慢阻肺,肺心病患者的心脏功能,有利于肺心病的抢救治疗。
In this paper, 60 patients with chronic obstructive pulmonary disease with left heart function were initially observed by echocardiography. The results suggest that COPD patients have significantly lower SVI, CI, and EF than those at baseline even at remission (P <0.001), and decrease in cardiac output in patients with COPD when combined with heart failure and respiratory failure More obvious. At the same time, it was observed that the EDV and MVCF in patients of pulmonary heart group were lower than that of the control group, suggesting that the decrease of left ventricular preload and the decrease of myocardial contractility may be the main reason for the decrease of cardiac output in patients with pulmonary heart disease. In addition, it was observed in 4 patients with acute respiratory failure under positive mechanical ventilation that stroke volume decreased with the increase of mechanical ventilation. Echocardiography could help us correctly estimate COPD, Coronary heart disease patients with heart function, is conducive to the treatment of pulmonary heart disease.
