目的 总结用自行研制的脑胶质瘤放疗囊,对恶性胶质瘤进行瘤腔间质放疗的长期疗效。方法 在理论计算和模拟测定的基础上,对25例恶性胶质瘤患者在手术切除肿瘤后的残瘤腔内放置放疗囊,术后经皮穿刺向化疗囊内注入740~1 110 MEq/次的~(125)I,对残瘤细胞进行长时间、局部高剂量的近距离照射。结果 模拟测定放射源1~2cm范围内的吸收剂量为30.4~91.3cGy/h,是常规经颅照射剂量的3~5倍。本组病例生存期明显延长,Kamofsky积分提高。病理证实近放射源处无肿瘤细胞增殖。结论 利用放疗囊对胶质瘤进行的~(125)I近距离放疗是一种无辐射污染、增减放射源简便,可对局部残瘤进行长周期、高剂量放疗的新方法。
Objective To summarize the long-term effect of tumor-interstitial radiotherapy on malignant gliomas with self-developed glioma radiotherapy capsule. Methods On the basis of theoretical calculation and simulation, 25 patients with malignant gliomas were treated with radiotherapy in the residual tumor cavity after surgical resection of tumor. Percutaneous puncture was performed after intraperitoneal puncture to inject 740-1 110 MEq / Of ~ (125) I, the residual tumor cells for a long time, local high-dose close-range irradiation. Results The absorbed dose in the range of 1 ~ 2 cm from simulated sources was 30.4 ~ 91.3 cGy / h, which was 3 ~ 5 times of that of conventional transcranial irradiation. The survival of patients in this group was significantly prolonged, Kamofsky points increased. Pathology confirmed near tumor cells without radioactive proliferation. CONCLUSION: Radiotherapy balloon is a new method of long-period and high-dose radiotherapy for local residual tumor with 125 I brachytherapy of glioma.
