目的 验证胶原酶盘外溶核的有效性,观察对正常椎间盘和神经根的影响。方法 将健康家兔64只随机等分成四组(n=16)即椎间盘突出注射胶原酶组(A组);注射NS组(B组);椎间盘未突出注射胶原酶组(C组);注射NS组(D组)。A、C组分别向硬膜外腔注射胶原酶300u(NS0.5ml)。BD组分别向硬膜外腔注射等量NS,分别于1、7、15、30d后测定坐骨神经传导速度(NCV),并行椎间盘及神经根的组织学观察。结果 A组术后各时间突出的髓核都有不同程度的溶解。B组术后各时间神经根有不同程度损伤,无椎间盘溶解的征象。A、C组NCV术后7、15d增快(P<0.01)。结论 硬膜外注射胶原酶确能溶解突出的椎间盘。胶原酶对正常椎间盘和神经根没有破坏作用,可使根性坐骨神经NCV增快。
Objective To verify the availability of collagenase extranodal discoid nucleus and observe the effect on normal disc and nerve root. Methods Sixty-four healthy rabbits were randomly divided into four groups (n = 16), ie collagenase group (group A), NS group (group B), collagenase group (group C) NS group (D group). A and C groups were injected with 300u collagenase (NS0.5ml) into the epidural space respectively. The rats in BD group were injected NS into the epidural space, respectively. Sciatic nerve conduction velocity (NCV) and histological observation of parallel intervertebral disc and nerve root were performed at 1, 7, 15 and 30 days respectively. Results A group of nucleus pulposus prominent at different times after operation had different degrees of dissolution. In group B, the nerve roots were damaged to varying degrees after operation, with no evidence of disc lysis. The NCV of group A and C increased at 7 and 15 days after operation (P <0.01). Conclusion Epidural injection of collagenase does dissolve the prominent disc. Collagenase on the normal disc and nerve root no destructive effect, can make the root of the sciatic nerve NCV faster.
