目的 观察单侧声带麻痹患者嗓音声学特征 ,探讨计算机声学测试分析方法在该病诊断及治疗中应用价值。方法 采用计算机嗓音声学测试系统对 5 0例正常成人及 30例单侧声带麻痹患者持续元音 /a声信号进行检测 ,对其主要声学参数 (jitter、shimmer、NNE)及声谱图特征 (谐波、共振峰、噪声成分 )进行分析 ,并比较以不同麻痹声带位置的声学特征 ,观察了杓状软骨拨动治疗前后嗓音声学性质变化。结果 单侧声麻痹嗓音各项声学参数值均较正常值明显增高 ,差别有显著意义 ;声谱图表现中高频区谐波及共振峰声能减弱或消失 ,噪声成分增加。麻痹声带位置直接影响嗓音声学性质 ,NNE更能客观反映麻痹声带的位置及声门闭合程度。杓状软骨拨动后各项声学参数均下降 ,声谱图中高频区声能有所恢复 ,噪声成分减少。结论 计算机声学分析各项参数均可作为评价声带麻痹嗓音损害程度的定量指标 ,NNE可作为客观反映麻痹声带的位置及声门闭合程度的定量参数。声谱图则以视觉图象形式显示了声带麻痹嗓音的声学特征 ,和声学参数结合 ,两者均可有效反映声带麻痹病变程度及声门闭合状态 ,有助于这一疾病疗效及预后的客观评价
Objective To observe the voice acoustic characteristics of patients with unilateral vocal cord paralysis and to explore the value of computer acoustics test and analysis in the diagnosis and treatment of the disease. Methods The continuous vowel / a acoustic signals of 50 normal adults and 30 unilateral vocal cord paralysis patients were detected by computer voice acoustics test system. The main acoustic parameters (jitter, shimmer, NNE) and sonographic features Harmonic, resonance, noise) were analyzed. The acoustic characteristics of different paralyzed vocal cords were compared. The changes of acoustic characteristics before and after arytenoid cartilage dialysis were observed. Results The acoustic parameters of unilateral acoustic paralysis voice were significantly higher than the normal values, the difference was significant; the sound spectrum showed that the harmonic and formant sound energy in the high-frequency region weakened or disappeared, and the noise component increased. Paralyzed vocal cords directly affect the acoustic properties of voice, NNE objectively reflect the paralytic vocal cord position and the degree of glottis closure. The acoustic parameters of the arytenoid cartilage were decreased after the dialing of the arytenoid cartilage, the sound in the high-frequency area of the spectrogram could be recovered and the noise component decreased. Conclusions The parameters of computer acoustics analysis can be used as a quantitative index to evaluate the voice damage degree of vocal cord paralysis. NNE can be used as a quantitative parameter to objectively reflect the position of vocal paralysis and the degree of glottis closure. The acoustic chart shows the acoustic characteristics of the vocal cord paralyzed voice in the form of visual images, combined with the acoustic parameters, both of which can effectively reflect the degree of vocal cord paralysis and the glottis closed state, which is helpful to the objective and objective of the curative effect and prognosis of the disease Evaluation
