目的探讨臂踝动脉脉搏波传导速度(baPWV)与慢性肾脏病(CKD)的相关性。方法采用横断面研究设计,对2010-2011年参加开滦集团健康体检且符合入选标准的5924人进行分析。CKD定义为尿蛋白阳性和(或)估算的肾小球滤过率(eGFR)<60mL/(min·1.73m2)。依据2010-2011年健康体检时baPWV的四分位数,将研究人群分为4组,采用多因素Logistic回归分析baPWV与CKD检出率的相关性。结果在纳入研究的5924人中,男性3709人(62.61%),年龄(57.2±11.9)岁,CKD的总检出率为10.75%。与baPWV第1四分位组相比,第2、第3和第4四分位组的血肌酐增高[(75.83±19.80)、(75.41±32.79)、(78.58±28.09)比(71.83±17.95)μmol/L,均P<0.01],eGFR降低[(92.33±16.47)、(89.16±16.78)、(82.65±17.82)比(96.05±16.56)mL/(min·1.73m2),均P<0.01],第3和第4四分位组CKD检出率增高(17.78%,10.56%比7.44%;均P<0.01)。多因素Logistic回归分析显示,校正年龄、性别、收缩压、心率、体质量指数、尿酸、空腹血糖、低密度脂蛋白胆固醇、总胆固醇、三酰甘油、高敏C反应蛋白、吸烟、饮酒及锻炼后,与第1四分位组比较,第4四分位组发生eGFR<60mL/(min·1.73m2)和CKD的OR(95%CI)值分别为2.60(1.17~5.80)和1.97(1.12~3.48)。结论 baPWV增加与CKD相关,高baPWV组人群CKD检出率增高。
Objective To investigate the relationship between brachial-ankle pulse wave velocity (baPWV) and chronic kidney disease (CKD). Methods A cross-sectional study design was used to analyze 5924 people who participated in the Kailuan Group’s physical examination from 2010 to 2011 and met the inclusion criteria. CKD was defined as urinary protein positive and / or estimated glomerular filtration rate (eGFR) <60 mL / (min · 1.73 m2). According to the quartiles of baPWV during physical examination from 2010 to 2011, the study population was divided into 4 groups. The correlation between baPWV and CKD detection rate was analyzed by multivariate Logistic regression. Results Of the 5924 people enrolled in the study, there were 3,709 males (62.61%) with an age of (57.2 ± 11.9) years and a total detection rate of 10.75% for CKD. Compared with the baPWV first quarter, the serum creatinine levels in the second, third and fourth quartiles increased (75.83 ± 19.80), (75.41 ± 32.79), (78.58 ± 28.09) vs (71.83 ± 17.95 (P <0.01) and eGFR decreased (92.33 ± 16.47), (89.16 ± 16.78), (82.65 ± 17.82) vs 96.05 ± 16.56 mL / (min · 1.73m2) ]. The detection rates of CKD increased in the third and fourth quartiles (17.78%, 10.56% vs 7.44%, all P <0.01). Logistic regression analysis showed that age, gender, systolic blood pressure, heart rate, body mass index, uric acid, fasting blood glucose, low density lipoprotein cholesterol, total cholesterol, triglyceride, high sensitivity C-reactive protein , And OR (95% CI) values of eGFR <60 mL / (min · 1.73m 2) and CKD were 2.60 (1.17 ~ 5.80) and 1.97 (1.12 ~ 3.48). Conclusions The increase of baPWV is associated with CKD, and the detection rate of CKD is increased in high baPWV group.
