目的 探讨紧邻肝内外重要血管的肝血管瘤的安全切除。方法 回顾性分析 1996～ 2 0 0 3年手术切除的 2 7例紧邻肝内外重要血管的肝血管瘤的临床资料。肝上下腔静脉、肝下下腔静脉、肝右静脉或肝中肝左静脉共干预置阻断带。使用多功能手术解剖器行刮吸法切除肿瘤。结果 2 7例肝血管瘤均安全切除。肝上下腔静脉、肝下下腔静脉、肝右静脉或肝中肝左静脉共干预置阻断带 ,成功完成下腔静脉裂口修补 2例 ,肝中静脉修补3例 ,肝右静脉修补 1例。结论 肝上、肝下下腔静脉和主肝静脉预置阻断带有利于复杂情况下的肝血管瘤的安全切除。
Objective To investigate the safety resection of hepatic hemangiomas adjacent to important blood vessels inside and outside the liver. Methods The clinical data of 27 cases of hepatic hemangiomas adjacent to the major intrahepatic and extrahepatic vessels surgically resected from 1996 to 2003 were retrospectively analyzed. Suprahepatic inferior vena cava, inferior vena cava, right hepatic vein or left hepatic vein in the liver. Scraping the tumor with a multifunctional surgical dissector. Results Twenty-seven hepatic hemangiomas were safely resected. The superior and inferior vena cava, inferior hepatic vein, right hepatic vein or left hepatic vein were intervened to block the inferior vena cava successfully. Two cases of inferior vena cava repair, three cases of middle hepatic vein repair and one case of right hepatic vein repair . Conclusion The suprahepatic, inferior hepatic inferior vena cava and the main hepatic vein prefabricated with safe removal of hepatic hemangiomas in complex cases.
