目的明确趋化因子受体CXCR4在非小细胞肺癌(NSCLC)组织的表达及其与TNM分期和纵隔淋巴结转移的关系,探索CXCR4作为肺癌患者预后指标的可行性。方法收集医院标本库中的肺癌标本(NSCLC组)61例,全部病人术前均未经过化疗或放疗,登记病人性别、年龄、组织学类型、TNM分期、是否淋巴结转移等临床资料,采用免疫组织化学(IHC)技术检测CXCR4的表达阳性率。对照组63例为正常肺组织(取自距肿瘤边缘5 cm以上处,经病理检查确证为不含有肿瘤细胞的正常肺组织)。结果NSCLC组中CXCR4阳性表达率为52.5%(32/61),对照组中CXCR4阳性表达率为17.5%(11/63),差异有统计学意义(χ~2=16.759,P<0.001)。CXCR4表达与肺癌TNM分期、纵隔淋巴结转移明显相关(P<0.05),而与肿瘤组织学类型、性别、年龄无关(P>0.05)。结论 CXCR4在NSCLC组织中高表达,与淋巴结转移、TNM分期紧密相关,在NSCLC生长、侵袭和转移中有着十分重要的作用。
Objective To determine the expression of chemokine receptor CXCR4 in non-small cell lung cancer (NSCLC) and its relationship with TNM staging and mediastinal lymph node metastasis, and to explore the feasibility of CXCR4 as a prognostic indicator in patients with lung cancer. Methods Sixty-one patients with lung cancer (NSCLC) were collected from the hospital specimen bank. All the patients were not subjected to chemotherapy or radiotherapy before surgery, and the clinical data such as patient’s sex, age, histological type, TNM stage and lymph node metastasis were collected. The positive rate of CXCR4 expression was detected by IHC. The control group of 63 cases of normal lung tissue (taken from the edge of the tumor more than 5 cm at the pathological examination confirmed that the normal lung tissue does not contain tumor cells). Results The positive rate of CXCR4 expression in NSCLC group was 52.5% (32/61). The positive rate of CXCR4 expression in control group was 17.5% (11/63), the difference was statistically significant (χ ~ 2 = 16.759, P <0.001). CXCR4 expression was significantly associated with TNM staging and mediastinal lymph node metastasis (P <0.05), but not with histological type, sex and age (P> 0.05). Conclusion CXCR4 is highly expressed in NSCLC tissues and closely related to lymph node metastasis and TNM stage. It plays a very important role in the growth, invasion and metastasis of NSCLC.
