改良营养膳食对住院重症监护室患者消化系统的影响

目的:探讨改良营养膳食对住院重症监护室(intensive care unit,ICU)患者消化系统的影响.方法:选取2014-01/12于天津市医科大学第二医院ICU收治的126例患者为研究对象,随机分为观察组和对照组,每组63例.全部患者均通过鼻胃管给予早期肠内营养,对照组采用能全素,观察组采用改良营养膳,其由三九复合膳及三九蛋白膳以2∶1混合而成,比较两组鼻饲后腹泻、便秘、胃潴留等并发症的发生率,以及胃肠功能评分及患者满意度.结果:观察组鼻饲后腹泻、便秘、胃潴留等并发症的发生率均显著低于对照组(P<0.05).观察组给予肠内营养后3、7 d的胃肠道评分均显著低于组内肠内营养前(P<0.05).出院时,观察组患者的满意度状况及总满意率均显著优于对照组(P<0.05).结论:改良营养膳可显著降低ICU患者胃肠功能障碍的发生风险,促进消化系统功能的恢复,提高了患者满意度,值得临床推广应用. Objective: To investigate the effect of modified nutritious diet on the digestive system of patients with intensive care unit (ICU) .Methods: 126 patients admitted to the ICU of the Second Hospital of Tianjin Medical University from January to December 2014 were enrolled in this study. Randomly divided into observation group and control group, 63 cases in each group.All patients were given early enteral nutrition by nasogastric tube, the control group with the use of vegan, the observation group with improved nutrition diet, which consists of 39 combined diets and 39 The protein diet was made by mixing 2: 1, and the complication rates of diarrhea, constipation and gastric retention were compared between the two groups after nasal feeding, as well as the gastrointestinal function score and patient satisfaction.Results: The diarrhea, constipation and gastric retention (P0.05) .The gastrointestinal scores of the observation group at 3 and 7 days after enteral nutrition were significantly lower than those before enteral nutrition (P <0.05). At discharge from hospital, the satisfaction degree and total satisfaction of patients in observation group were significantly better than those in control group (P0.05) .Conclusion: Modified nutrition diet can significantly reduce the risk of gastrointestinal dysfunction in ICU patients and promote the recovery of digestive system function , Improve patient satisfaction, it is worth clinical promotion should use.
