内窥镜进行胰胆管造影(ERCP)能显示出胰管和胆管的形态,已被广泛应用,但不能显示胰实质或间质的改变。本文作者改进了造影剂,采用ERCP的方法,使胰实质造影成功,并将此法称为内窥镜逆行胰实质造影法(ERPP)。ERPP采用的内窥镜和术前准备与ERCP相同,在操作时须将造影导管插至胰管内,并以一定压力推注造影剂。因胰实质造影象在数秒至10秒即消失,为取得良好造影图象,可取左下侧卧位,在注射造影剂后即连续摄片。为增加造影剂对胰管上皮的通透性,在水溶性造影剂碘酞氨酸钠(Sodium iothalamate)内加入一种表面活性剂聚氧乙烯氢化蓖麻油(Polyo-xyelene hydrogenated castor oil,Hco-50、Hco-
