目的探讨应用红细胞分布宽度(RDW)判断乳腺癌转移的价值。方法选择本院2013年7月-2015年1月初诊为乳腺癌的患者332例,根据临床分期分组:1期+2期88例,3期108例,4期136例;根据有无淋巴结转移分组:有转移271例,无转移61例;根据有无远端转移分组:有转移136例,无转移196例。所有受试者检测RDW水平,应用ROC曲线判断RDW提示乳腺癌转移的价值。结果 4期乳腺癌患者RDW水平高于3期、1期+2期患者,3期高于1期+2期,组间差异有统计学意义(P<0.01);有淋巴结转移者RDW水平高于无淋巴结转移者,差异有统计学意义(P<0.01);有远端转移者RDW水平高于无远端转移者,差异有统计学意义(P<0.01)。RDW判断乳腺癌患者有无淋巴结转移ROC曲线的AUC为0.909,cut-off值为14.85,敏感度为70.8%,特异度为95.1%。RDW判断乳腺癌患者有无远端转移ROC曲线的AUC为0.848,cut-off值为15.65,敏感度为73.5%,特异度为80.1%。结论 RDW能较准确地提示乳腺癌转移,可以作为潜在的乳腺癌转移标志物,值得深入研究。
Objective To investigate the value of using RDW to determine the metastasis of breast cancer. Methods A total of 332 patients with newly diagnosed breast cancer from July 2013 to January 2015 in our hospital were enrolled. According to the clinical stage, 88 cases were stage I + 2, 108 cases were stage III and 136 cases were stage IV. According to the presence or absence of lymph node metastasis Grouping: There were 271 cases with metastasis and 61 cases without metastasis. There were 136 cases with metastasis and 196 cases without metastasis according to the presence or absence of distant metastasis. All subjects tested RDW levels and used ROC curves to determine the value of RDW in breast cancer metastasis. Results The level of RDW in patients with stage 4 breast cancer was higher than that in stage 3, stage 1 and stage 2, stage 3 was higher than stage 1 and stage 2, the difference was statistically significant (P <0.01); RDW with lymph node metastasis was high There was a significant difference between the two groups (P <0.01). There was a statistically significant difference in the RDW level between patients without lymph node metastasis and without distant metastasis (P <0.01). The AUC of RDW to determine the ROC curve of lymph node metastasis in patients with breast cancer was 0.909, the cut-off value was 14.85, the sensitivity was 70.8% and the specificity was 95.1%. RDW to determine whether the distal metastasis ROC curve of patients with AUC of 0.848, cut-off value of 15.65, sensitivity was 73.5%, specificity was 80.1%. Conclusion RDW can prompt the metastasis of breast cancer more accurately, which can be used as a potential marker of breast cancer metastasis. It is worth further study.
