县级政府是中国当下最完整的一级地方政府,在具体政策的落实过程中,它既是接受上级政策的执行者,又是因地制宜的政策设计者。本文以XR县为例,分析了县级政府在地方医疗卫生服务改革中的角色、行为与激励机制,县级政府通过“院财局管”、“乡村一体化”以及“干部召回”等管理模式创新,上收了行政权限,使得在分权化的行政体制下,有效地推动了地方卫生服务改革。但公共服务政策执行的结果并非是一个“共赢”局面,在资源重新布局中,资源越发向上集中,地方公共服务体系并未获得实质上的改善。
At the county level, the government is the most complete local government at present. In the process of implementing specific policies, it is not only the executor who accepts the policy of the higher level but also the policy designer who adapts to the local conditions. Taking XR county as an example, this paper analyzes the role, behavior and incentive mechanism of the county government in the reform of local health care services. The county government through the “hospital management”, “rural integration” and “ Cadres recall ”and other management innovation, the collection of administrative authority, making decentralized administrative system, effectively promote the reform of local health services. However, the result of the implementation of the public service policy is not a “win-win” situation. In the re-layout of resources, resources are increasingly concentrated and the local public service system has not been substantially improved.
