目的探讨子痫前期孕妇脐动脉血流指数(S/D)与红细胞比容(HCT)、D-二聚体和血小板最大聚集率的相关性及对妊娠并发症和围生儿不良结局的预测价值。方法选择2015年1月-2017年1月西宁市第三人民医院收治的80例子痫前期患者为子痫前期组(轻度48例和重度32例),80例正常孕妇为对照组。分析子痫前期组S/D与HCT、D-二聚体及血小板最大聚集率的相关性;比较两组的妊娠结局。结果与对照组相比,子痫前期组的S/D、HCT、D-二聚体及血小板最大聚集率均明显较高(均P<0.05);子痫前期组S/D与HCT(r=0.68)、D-二聚体(r=0.82)及血小板最大聚集率(r=0.73)均呈明显正相关(均P<0.05)。与对照组相比,子痫前期组妊娠并发症及围生儿不良结局发生率均明显较高;S/D异常者不良妊娠结局的发生率高于S/D正常者。结论子痫前期孕妇脐动脉S/D与HCT、D-二聚体及血小板最大聚集率均呈显著的正相关,S/D异常可增加发生妊娠并发症及围生儿不良结局的风险。
Objective To investigate the correlation between umbilical artery blood flow index (S / D) and hematocrit (HCT), D-dimer and platelet aggregation rate in pregnant women with preeclampsia and the prediction of adverse pregnancy outcome and perinatal outcome value. Methods From January 2015 to January 2017, 80 patients with preeclampsia in Xining Third People’s Hospital were treated with preeclampsia (48 mild cases and 32 severe cases), and 80 normal pregnant women as control group. The correlation between S / D and HCT, D-dimer and platelet aggregation in preeclampsia was analyzed. Pregnancy outcome was compared between the two groups. Results Compared with the control group, the maximum aggregation rate of S / D, HCT, D-dimer and platelet in preeclampsia group were significantly higher (all P <0.05) = 0.68), D-dimer (r = 0.82) and platelet aggregation rate (r = 0.73) (all P <0.05). Compared with the control group, preeclampsia group pregnancy complications and perinatal unacceptable incidence were significantly higher; S / D abnormalities were higher than the incidence of adverse pregnancy outcomes S / D normal. Conclusions There is a significant positive correlation between S / D of umbilical artery and HCT, D-dimer and the maximum aggregation rate of platelet in pregnant women with preeclampsia. S / D abnormality may increase the risk of pregnancy complications and perinatal adverse outcomes.
