目的:对椎体成形术(percutaneous vertebroplasty,PVP)与椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗骨质疏松性椎体压缩骨折的疗效进行对比研究。方法:将我院2014年1月~2015年6月收治的86例老年脊柱压缩性骨折患者随机分为PKP组与PVP组,PKP组43例患者行PKP治疗,PVP组43例患者行PVP治疗,对比两组患者的治疗效果。结果:两组患者的手术时间、骨水泥注入量、术后伤椎增加高度及术后12个月伤椎高度丢失有明显差异(P<0.05),两组患者术后VAS评分无明显差异(P>0.05)。结论:PKP与PVP治疗骨质疏松性椎体压缩骨折止痛效果均比较理想,PKP复位效果更好,但是术后椎体高度的再次丢失较为明显,临床治疗时应根据患者具体情况选择。
Objective: To compare the efficacy of percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) in the treatment of osteoporotic vertebral compression fractures. Methods: A total of 86 elderly patients with spinal compression fractures admitted to our hospital from January 2014 to June 2015 were randomly divided into PKP group and PVP group, 43 patients in PKP group received PKP, 43 patients in PVP group received PVP , Comparing the treatment effect of two groups of patients. Results: There were significant differences in the operation time, the amount of cement injection, the height of injured vertebrae and the height loss of injured vertebra 12 months after operation (P <0.05). There was no significant difference in postoperative VAS score between the two groups P> 0.05). CONCLUSION: PKP and PVP are effective in treating osteoporotic vertebral compression fractures. The effect of PKP reduction is better, but the loss of vertebral body height after surgery is more obvious. The clinical treatment should be based on the specific conditions of patients.
