人脑平均重量约为体重的2%,耗氧量却占全身耗氧量的20%,脑血流量(CBF)50毫升/100克/分。脑组织的代谢主要依靠葡萄糖的有氧氧化,但无葡萄糖的储备,必须依靠血液的持续供应才能维持大脑的正常功能。CBF降至10毫升/100克/分以下时,即发生脑细胞不可逆性损伤。血液流变学、局部微循环流态及其有形成分的聚集,血pH及局部灌注压的改变均与梗塞区CBF密切相关。CBF的改善与否亦决定其预后。
