目的:探讨Stanford B型主动脉夹层主动脉腔内修复术(TEVAR)术后计算机断层扫描(CT)在随访中的影像学变化及其应用价值。方法:回顾性分析广东省中山市人民医院2014年1月至2015年12月收治的33例主动脉夹层患者,经TEVAR治疗后,随诊两年的CT成像对支架位置、形态、通畅度、支架内漏、远端破口、主动脉重塑等情况的观察。结果:33例术后CT可显示:支架位置、形态、通畅良好33例,支架无内漏,存在远端破口33例,支架源性新破口(SINE)2例,近端夹层内血栓形成33例。术后夹层扩张2例,缩小17例,不变14例。结论:EVAR术后CT可通过多种图像重建,为主动脉影像学提供大量信息,是一种无创、快速、安全及准确的评价主动脉夹层情况的方法,可作为是主动脉夹层术后随访的首选检査方法。
Objective: To investigate the imaging changes and its application value of computed tomography (CT) at Stanford type B aortic dissection after endovascular repair (TEVAR). Methods: A retrospective analysis of 33 patients with aortic dissection who were treated in Zhongshan People’s Hospital of Guangdong Province from January 2014 to December 2015 was performed. After two-year CT scan of TEVAR, the location, shape, patency, Stent leakage, distal break, aortic remodeling and other observations. Results: Thirty-three cases of postoperative CT showed that there were 33 cases with good stent placement and shape, no internal leakage, 33 cases with distal breakage, 2 cases with SINE, 2 cases with proximal intra-arterial thrombus Form 33 cases. Postoperative dissection in 2 cases, reduced in 17 cases, unchanged in 14 cases. Conclusion: The post-EVAR CT scan is a non-invasive, rapid, safe and accurate method to evaluate the aortic dissection through a variety of image reconstruction, providing a large amount of information for aortic imaging. It can be used as a follow-up after aortic dissection The preferred method of inspection.
