目的探讨同型半胱氨酸(HCY)和叶酸(FA)及维生素B12(VB12)在2型糖尿病肾病(T2DN)中的变化。方法根据24小时尿白蛋白排泄率(urinary albumin excretion rate,UAER)将122例2型糖尿病(T2DM)患者分为单纯糖尿病(SDM)组、早期糖尿病肾病(EDN)组和临床糖尿病肾病(CDN)组,50例体检健康者作为健康对照组分别检测同型半胱氨酸和叶酸及维生素B12水平。结果与健康对照组比较,T2DM3组患者HCY水平升高(P<0.05)而叶酸和维生素维生素B12水平降低(P<0.05),HCY与FA和VB12均呈负相关,r分别为-0.61和-0.43(P<0.01)。结论2型糖尿病肾病患者血清HCY水平随着UAER的增加而升高,FA和VB12水平随着UAER的增加而下降,这些指标可能与T2DN的病情进展有关。
Objective To investigate the changes of homocysteine (HCY), folic acid (FA) and vitamin B12 (VB12) in type 2 diabetic nephropathy (T2DN). Methods 122 patients with type 2 diabetes mellitus (T2DM) were divided into simple diabetic group (SDM), early diabetic nephropathy (EDN) group and clinical diabetic nephropathy (CDN) according to urinary albumin excretion rate (24 hours) Group and 50 healthy people as healthy control group were detected homocysteine and folic acid and vitamin B12 levels. Results Compared with healthy control group, HCY level in T2DM3 group was significantly increased (P <0.05) and folic acid and vitamin B12 levels were decreased (P <0.05), HCY was negatively correlated with FA and VB12, r was -0.61 and - 0.43 (P <0.01). Conclusions Serum HCY levels in patients with type 2 diabetic nephropathy increase with the increase of UAER. The levels of FA and VB12 decrease with the increase of UAER. These indexes may be related to the progression of T2DM.
