目的采用常规细胞学检查和细胞DNA定量分析技术进行宫颈癌筛查,以组织学诊断为金标准,评估细胞DNA定量分析诊断技术在筛查宫颈癌中应用的敏感性、特异性和准确性。方法制片采用液基滴片技术,每个标本制成两张薄层细胞学标本片,其中一张进行巴氏染色做常规细胞学诊断,另外一张进行Feulgen染色做细胞DNA定量测定。结果 4000例中3820例DNA倍体检测阴性,通过细胞DNA定量分析方法在筛查发现宫颈病变的敏感性为81%,宫颈病变特异性为72%,而常规细胞学方法为51%和90%。结论对同一个标本采用不同的筛查方法进行评估,比较常规细胞学与细胞DNA定量分析技术在宫颈癌筛查中的应用成效。DNA检测也可以作为一个质控方法核实常规细胞学的结果,保证细胞学诊断的准确性。
OBJECTIVE: To screen cervical cancer by routine cytology and cell DNA quantitative analysis, and to evaluate the sensitivity, specificity and accuracy of cell DNA quantitative analysis in screening cervical cancer by using histological diagnosis as the gold standard. Methods Using liquid-based technique, each specimen was made into two thin-layer cytology specimens, one of which was used for routine cytology diagnosis by Papanicolaou and the other for Feulgen staining for DNA quantification. Results 3800 DNA samples were negative in 4,000 samples. The sensitivity of screening cervical lesions was 81% by DNA quantitative analysis and 72% by cervical cytology. The conventional cytology methods were 51% and 90% . Conclusion Different screening methods were used to evaluate the same specimen. The effectiveness of routine cytology and DNA quantitative analysis in cervical cancer screening was compared. DNA testing can also be used as a quality control method to verify routine cytology results, to ensure the accuracy of cytological diagnosis.
