目的:探讨丙型肝炎病毒抗体(antiHCV)阳性人群血清肝炎病毒核糖核酸(HCVRNA)和丙氨酸氨基转移酶(ALT)的关系。方法:取334例抗HCV阳性患者血清,采用酶联免疫吸附试验(ELISA)法检测抗HCV,荧光定量PCR法检测HCVRNA,全自动生化检测仪测定ALT水平,观察HCVRNA阳性组和HCVRNA阴性组患者血清ALT水平,同时对HCVRNA低载量组、HCVRNA中载量组及HCVRNA高载量组的ALT水平异常率进行Logist回归分析。结果:在抗HCV阳性血清样本(334例)中,HCVRNA载量阳性226例(67.7%),HCVRNA阴性108例(32.3%);HCVRNA阳性组患者ALT水平和异常率均高于HCVRNA阳性组患者(P<005);HCVRNA低载量组、HCVRNA中载量组及HCVRNA高载量组的ALT异常率分别为33.3%、69.5%、74.4%,ALT均值分别为100.7、103.0及98.6U/L;Logist回归分析结果显示,HCVRNA载量与ALT异常率呈正相关(r=0.874,P<0.05);HCVRNA载量与ALT水平变化无明显相关性(r=0.469,P>0.05)。结论:ALT异常率与HCVRNA载量呈正相关,而ALT水平与HCVRNA载量无相关性。
Objective: To investigate the relationship between serum hepatitis C virus RNA (HCVRNA) and alanine aminotransferase (ALT) in hepatitis C virus (HCV) positive population. Methods: Sera of 334 anti-HCV positive patients were taken, anti-HCV was detected by enzyme-linked immunosorbent assay (ELISA), HCV RNA by real-time quantitative PCR, ALT levels were measured by automatic biochemical analyzer, HCV RNA positive group and HCVRNA negative ALT levels in patients with low serum HCVRNA, HCVRNA in high-load group and HCVRNA high-load group ALT level Logistic regression analysis. Results: Among 236 anti-HCV positive serum samples, 226 (67.7%) were positive for HCV RNA and 108 (32.3%) were negative for HCVRNA; the levels of ALT and abnormality in HCVRNA-positive group were higher than those in HCVRNA-positive group (P <005). The ALT abnormalities in HCV RNA low load group, HCV RNA medium load group and HCVRNA high load group were 33.3%, 69.5%, 74.4%, respectively. The mean ALT were 100.7, 103.0 and 98.6 Logistic regression analysis showed that there was a positive correlation between HCV RNA load and ALT abnormality rate (r = 0.874, P <0.05). There was no significant correlation between HCV RNA load and ALT level (r = 0.469, P> 0.05). Conclusion: The abnormal rate of ALT is positively correlated with the HCV RNA load, while the ALT level has no correlation with the HCV RNA load.
