目的应用典则相关分析探讨急性心肌梗死合并室壁瘤形成患者血浆肌钙蛋白I(TnI)、脑钠素(BNP)、白介素-8(IL-8)与心室收缩功能、舒张功能、收缩同步性的相关性。方法84例首次急性前壁心肌梗死且左室造影证实存在室壁瘤形成的患者,胸痛发作12小时测定TnI、BNPI、L-8。入院后第1周应用平衡法核素心室造影(ERNA)测定左室收缩功能(LVSF)、舒张功能(LVDF)、收缩同步性功能(LVSS)各参数。应用典则相关分析,分析TnI、BNP、IL-8与LVSF、LVDF、LVSS的相关性。结果3对典则变量的典则相关系数分别为0.769253(P=0.0373)、0.662367(P=0.0418)、0.331153(P=0.382),第1对(V1,W1)、第2对(V2,W2)典则变量共代表了94.77%的相关信息。从标准化典则相关系数可知,V1典则变量主要反映了BNP、IL-8,W1主要反映了峰相位标准差(PSD)、峰驻盈时间(TPFR)、峰充盈率(PFR)。TnII、L-8、BNP与PSD、TPFR呈正相关,与PFR呈负相关。V2典则变量主要反映TnI,W2主要反映LVEF、半高宽(FWHM)。TnI与FWHM呈正相关,与LVEF呈负相关。结论血浆TnI、IL-8、BNP升高可以作为急性心肌梗死合并室壁瘤形成患者心室收缩功能、舒张功能、收缩同步性降低的预测因子。
Objective To investigate the relationship between plasma Troponin I (TnI), brain natriuretic peptide (BNP), interleukin-8 (IL-8) and ventricular systolic function, diastolic function and systolic in patients with acute myocardial infarction Sexual relevance. Methods Eighty-four patients with acute anterior myocardial infarction and left ventricular angiography confirmed the presence of aneurysm. TnI, BNPI and L-8 were measured 12 hours after onset of chest pain. The first week after admission, the parameters of left ventricular systolic function (LVSF), diastolic function (LVDF), systolic synchrony function (LVSS) The association between TnI, BNP, IL-8 and LVSF, LVDF and LVSS was analyzed using canonical correlation analysis. Results The canonical correlation coefficients of the three pairs of canonical variables were 0.769253 (P = 0.0373), 0.662367 (P = 0.0418), 0.331153 (P = 0.382), the first pair (V1, W1) The canonical variables represent 94.77% of the relevant information. From the Canonical Canonical Correlation Coefficients, we can see that V1 canonical variables mainly reflect BNP, IL-8 and W1 mainly reflect the peak phase standard deviation (PSD), peak transit time (TPFR) and peak charge ratio (PFR). TnII, L-8 and BNP were positively correlated with PSD and TPFR, but negatively correlated with PFR. V2 canonical variables mainly reflect TnI, W2 mainly reflect LVEF, FWHM. TnI was positively correlated with FWHM and negatively correlated with LVEF. Conclusions The increase of plasma TnI, IL-8 and BNP can be used as predictors of ventricular systolic function, diastolic function and systolic synchrony in patients with acute myocardial infarction with aneurysm.
