为评价急相蛋白、白细胞介素6(IL-6)、白细胞介素8(IL-8)对极低出生体重儿(VLBWI)细菌感染早期诊断价值,对1998年12月~2000年4月收治的42例VLBWI,按细菌感染的诊断标准分感染组23例(8例败血症、15例可疑败血症)和无感染组19例,用散射比浊法及双抗体夹心ELLSA法,于入院当天、第3天、第5天分别检测血清中C反应蛋白(CRP)、结合珠蛋白(HP)及IL-6、IL-8水平。检测结果的比较显示:对VLBWI细菌感染诊断敏感度IL-6为86.96%,CRP为73.91%,IL-8为69.56%。血清IL-6水平与CRP和HP呈正相关,r=0.425,P<0.05和r=0.545,P<0.01。结果提示血甭IL-6有较高敏感性,而CRP敏感性稍低但特异性高(89.47%),且两者有紧密内在联系。因此,故血清IL-6和CRP水平可作为VLBWI细菌感染早期诊断首选指标之一。
To evaluate the early diagnostic value of acute phase protein, interleukin 6 (IL-6) and interleukin 8 (IL-8) in bacterial infection of very low birth weight infants (VLBWI) Twenty-four patients with VLBWI were divided into two groups: 23 cases (8 cases of sepsis, 15 cases of suspected sepsis) and 19 cases of non-infected group according to the diagnostic criteria of bacterial infection. Using the nephelometry and double antibody sandwich ELLSA method, on the day of admission, Serum levels of C-reactive protein (CRP), haptoglobin (HP), IL-6 and IL-8 were detected on the 3rd and 5th days respectively. The comparison of test results showed that the sensitivity of the detection of bacterial infection of VLBWI was 86.96%, the CRP was 73.91%, and the IL-8 was 69.56%. Serum IL-6 levels were positively correlated with CRP and HP, r = 0.425, P <0.05 and r = 0.545, P <0.01. The results suggest that blood 甭 IL-6 has a higher sensitivity, while CRP sensitivity is slightly lower but high specificity (89.47%), and the two are closely related. Therefore, the level of serum IL-6 and CRP can be used as one of the primary indicators of early diagnosis of bacterial infection of VLBWI.
