目的分析湖南省2011年艾滋病抗病毒治疗的情况,了解该省艾滋病患者治疗的现状及存在的问题。方法从国家艾滋病综合防治信息系统中导出湖南省患者治疗数据,用SPSS 17.0统计软件进行分析处理,了解患者在接受治疗时的WHO分期、基线CD4+T淋巴细胞水平、治疗与确证时间间隔及相关临床症状与疾病。结果 2011年共有1 474例成人病例纳入治疗,平均年龄(42.0±13.1)岁,66.1%(975例)是男性,感染途径以性传播为主(81.5%,1 202例);接受治疗时,38.5%(568例)的患者处于WHO临床Ⅰ期(无症状期),24.6%(362例)的患者处于临床Ⅱ期(轻度疾病期),24.2%(357例)的患者处于临床Ⅲ期(中度疾病期),12.7%(187例)的患者处于临床Ⅳ期(严重疾病期);基线CD4+T淋巴细胞平均166个/μl,<50个/μl、51~200个/μl、201~350个/μl及>350个/μl的分别占22.7%、37.5%、37.1%和2.7%;74.3%的基线CD4+T淋巴细胞计数低于200个/μl的患者能在确证诊断后6个月内接受治疗,治疗与确证诊断间隔中位数为70d;接受治疗前的1年有175例(11.9%)发生结核感染,563例(38.2%)在治疗前3个月内出现1种以上的症状和疾病,较常见的是发热(17.1%)、皮肤损害(9.1%)和鹅口疮(7.1%)。结论患者接受抗病毒治疗前CD4水平较低,部分已进入疾病期,需要进一步加强早期检测,使更多患者能够在疾病早期得到确诊并及时纳入治疗。
Objective To analyze the situation of AIDS antivirus treatment in Hunan Province in 2011 and understand the current situation and existing problems of AIDS treatment in this province. Methods The treatment data of patients in Hunan Province were derived from the national AIDS integrated prevention and control information system and analyzed by SPSS 17.0 statistical software to understand the WHO staging, the baseline CD4 + T lymphocyte level, the time interval between treatment and confirmation, and the correlation Clinical symptoms and diseases. The average baseline CD4 + T lymphocyte count was 166 / μl, <50 / μl, 201 to 350 cells / μl and> 350 cells / μl accounted for 22.7%, 37.5%, 37.1% and 2.7% respectively; patients with a baseline CD4 + T lymphocyte count of less than 200 cells / μl in 74.3% Within 6 months of treatment, the median interval between diagnosis and diagnosis was 70 days. In the 1 year before treatment, 175 cases (11.9%) had tuberculosis infection, and 563 cases (38.2%) appeared 1 month before treatment More common types of symptoms and illnesses are fever (17.1%), skin damage (9.1%) and thrush (7.1%). Conclusions The CD4 level in patients receiving antiretroviral therapy is low and some have entered the disease stage. Early detection needs to be further strengthened so that more patients can be diagnosed and treated promptly in the early stage of the disease.
