目的探讨不同剂量三维适形放疗对恶性肿瘤患者凝血功能的影响。方法 30例恶性肿瘤患者(放疗组)行三维适形放疗,分别于放疗前及放疗20、40、60Gy时测PT、APTT、TT、FIB、D-D、PLT含量。另择健康体检者30例作为健康对照组。比较2组放疗前及放疗不同剂量时凝血指标的变化。结果与健康对照组比较,放疗组PT、APTT、TT差异无统计学意义(P>0.05),PLT、FIB、D-D显著升高(P<0.05)。与放疗前比较,放疗组放疗后PT、APTT、TT变化差异无统计学意义(P>0.05),PLT明显减少,且随剂量的增加而减少(P<0.05);D-D、FIB明显增加,且随剂量的增加而增加(P<0.05)。结论不同剂量三维适形放疗对恶性肿瘤凝血功能产生一定影响,随剂量增加凝血功能障碍越明显。
Objective To investigate the effect of different doses of three-dimensional conformal radiotherapy on coagulation function in patients with malignant tumor. Methods Thirty patients with malignant tumor (radiotherapy group) underwent three-dimensional conformal radiotherapy. The levels of PT, APTT, TT, FIB, D-D and PLT were measured before radiotherapy and at 20, 40 and 60 Gy respectively. Another 30 healthy subjects were selected as healthy control group. The changes of coagulation index in two groups before radiotherapy and at different doses of radiotherapy were compared. Results Compared with healthy control group, PT, APTT and TT in radiotherapy group had no significant difference (P> 0.05), and PLT, FIB and D-D increased significantly (P <0.05). Compared with those before radiotherapy, there was no significant difference in PT, APTT and TT after radiotherapy (P> 0.05), PLT decreased obviously and decreased with the increase of dose (P <0.05); DD and FIB increased obviously Increased with dose (P <0.05). Conclusion Different doses of three-dimensional conformal radiotherapy have a certain impact on the coagulation function of malignant tumors, the more obvious the coagulation dysfunction increases with the dosage.
