目的:探讨慢性淋巴细胞白血病(CLL)细胞遗传学及免疫表型特征与预后的关系,为其诊断及治疗提供依据。方法:应用常规R显带技术及流式细胞术对45例CLL患者分别进行细胞遗传学和免疫表型分析。结果:45例CLL病例中,染色体异常核型检出率为31.1%(14/45),共检出8种主要异常核型,分别为+12、14q+、13q-、t(11;14)、del(11q)、6q-、20p-、+mar;免疫表型结果为均高表达HLA-DR(100%)和CD19(100%),CD5(95.6%),其次为CD23(93.3%),CD20(91.1%),CD22(68.9%),CD38(20.0%),无1例表达CD10,其中CD5+CD19+双阳性者43例。结论:常规细胞遗传学(CC)检测CLL染色体核型异常阳性率偏低;典型的CLL免疫表型为HLA-DR+、CD5+、CD19+、CD23+;细胞遗传学结合免疫表型对CLL诊断及预后有重要价值。
Objective: To investigate the relationship between cytogenetics and immunophenotypic features of chronic lymphocytic leukemia (CLL) and prognosis and to provide evidence for its diagnosis and treatment. Methods: Cytogenetics and immunophenotyping were performed in 45 patients with CLL using routine R-banding technique and flow cytometry. Results: Among the 45 cases of CLL, the detection rate of chromosomal abnormalities was 31.1% (14/45). Eight major abnormal karyotypes were detected, which were +12, 14q +, 13q-, t (11; 14) (100%) and CD19 (100%), CD5 (95.6%), followed by CD23 (93.3%). The results of immunophenotyping showed that the expression of HLA- , CD20 (91.1%), CD22 (68.9%), CD38 (20.0%), none of them expressed CD10, of which 43 cases were CD5 + CD19 + double positive. CONCLUSION: The positive rate of CLL chromosome abnormalities detected by conventional cytogenetics (CC) is low. The typical CLL immunophenotypes are HLA-DR +, CD5 +, CD19 +, and CD23 +. Cytogenetics and immunophenotyping have a significant effect on the diagnosis and prognosis of CLL Important value.
