目的 观察急性心肌梗死后心室结构的主要变化及心肌局部血管紧张素Ⅱ和羟脯氨酸含量的变化 ,并观察黄芪的治疗作用。方法 采用兔制备心肌梗死模型 ,黄芪组自术后第二日腹腔注射黄芪 2~ 3ml,每日二次 ,共三周。于术后第六周处死动物 ,用精确天平及精密卡尺直接测量左室重量、全心重量及梗死区、游离壁和室间隔厚度。采集标本 ,检测梗死区、游离壁及正常兔心肌血管紧张素Ⅱ和羟脯氨酸含量。结果 梗死后全心重量及左室重量均增加 ,差异显著 ,室间隔及游离壁厚度增加 ,梗死区室壁变薄 ,差异显著 ;心肌梗死后心肌局部血管紧张素Ⅱ含量显著增加 ,梗死区羟脯氨酸含量显著增加 ,游离壁羟脯氨酸含量略有增加 ;经黄芪治疗后左室重量和全心重量均下降 ,其中全心重量尚未恢复至正常 ;心肌血管紧张素Ⅱ含量明显下降 ,梗死区羟脯氨酸含量略有下降。结论 心肌梗死后心室构型发生明显变化 ,血管紧张素Ⅱ和羟脯氨酸参与左室重构 ,黄芪有一定的治疗作用。
Objective To observe the main changes of ventricular structure and the changes of myocardial angiotensin Ⅱ and hydroxyproline contents after acute myocardial infarction and to observe the therapeutic effect of astragalus. Methods The rabbit model of myocardial infarction was prepared. Astragalus was injected intraperitoneally with 2 ~ 3ml astragalus twice a day for three weeks. Animals were sacrificed on the sixth week after surgery. Left ventricular mass, total heart weight and infarct size, free wall thickness and interventricular septum thickness were measured directly using a precision balance and a precision caliper. The specimens were collected to detect the content of angiotensin Ⅱ and hydroxyproline in infarcted area, free wall and normal rabbit myocardium. Results The total heart weight and left ventricular weight increased after infarction, with significant difference. The thickness of ventricular septum and free wall increased. The wall of infarct area became thinner and the difference was significant. The content of angiotensin Ⅱ in myocardium increased significantly after myocardial infarction, Proline content increased significantly, the free wall hydroxyproline content increased slightly; after treatment with astragalus, left ventricular mass and whole heart weight decreased, of which the heart weight had not returned to normal; myocardial angiotensin Ⅱ content decreased significantly, Hydroxyproline content in infarction area decreased slightly. Conclusions The changes of ventricular configuration after myocardial infarction have obvious changes. Angiotensin Ⅱ and hydroxyproline are involved in left ventricular remodeling and astragalus has a certain therapeutic effect.
