目的探讨癌蛋白metadherin(MTDH)在甲胎蛋白(AFP)阴性肝癌组织中表达的临床意义。方法采用免疫组织化学方法检测我院2007~2010年期间152例AFP阴性肝癌患者术后肝癌组织中MTDH蛋白表达,分析MTDH蛋白表达与AFP阴性肝癌患者临床病理特征的关系。预后分析采用Cox比例风险模型,采用KaplanMeier方法分析MTDH蛋白表达情况与生存的关系。结果 MTDH在AFP阴性肝癌组织中高表达率为60.53%(92/152)。MTDH蛋白高表达与肿瘤直径(P=0.029)、Edmondson分级(P=0.032)、微血管浸润(P=0.024)及肿瘤复发(P=0.014)有关。单因素和多因素结果均表明MTDH蛋白高表达与患者较差的生存率有关(P=0.002,P=0.017)。结论 MTDH蛋白在AFP阴性肝癌组织中存在高表达,MTDH蛋白高表达是影响AFP阴性肝癌患者根治术后生存较差的预后因素之一。
Objective To investigate the clinical significance of oncoprotein metadherin (MTDH) expression in AFP negative hepatocellular carcinoma. Methods Immunohistochemistry was used to detect the expression of MTDH protein in 152 HCC tissues from 2007 to 2010 in our hospital. The relationship between MTDH protein expression and the clinicopathological features of AFP negative HCC was analyzed. Prognosis Analysis Cox proportional hazards model was used to analyze the relationship between MTDH protein expression and survival by Kaplan Meier method. Results The high expression rate of MTDH in AFP negative HCC tissues was 60.53% (92/152). The high expression of MTDH protein was correlated with tumor diameter (P = 0.029), Edmondson classification (P = 0.032), microvascular invasion (P = 0.024) and tumor recurrence (P = 0.014). Univariate and multivariate results showed that high MTDH protein expression was associated with poorer survival (P = 0.002, P = 0.017). Conclusion The MTDH protein is highly expressed in AFP negative hepatocellular carcinoma. The high expression of MTDH protein is one of the prognostic factors that affect the survival of AFP negative hepatocellular carcinoma patients after radical operation.
