目的建立孕20~26周胎儿内踝角正常参考值范围,探讨内踝角在诊断马蹄内翻足中的价值。方法将孕20~26周产前超声筛查中可疑马蹄内翻足的42只胎足(共27胎)作为病例组,另随机选取20~26周产前超声筛查正常足300只(共150胎)作为正常对照组,分别测量两组胎儿第一、五跖骨横切面足底连线与胫骨长轴夹角(即内踝角)大小,对两组数据进行直线相关分析。结果正常胎儿内踝角各孕周测量值恒定,与孕周变化无显著相关性(P>0.05),(±s)为(78.89°±9.24°),病例组(±s)为(37.64°±7.93°),与正常对照组相比,内踝角测量值差异具有统计学意义(P<0.01)。结论正常胎儿内踝角的角度与孕周变化无显著相关性,测量值可为产前诊断马蹄内翻足提供较准确的诊断信息,具有较高的临床应用价值。
Objective To establish a normal reference range of fetal medial malleolus between 20 and 26 weeks of gestation and to explore the value of medial malleolus angle in the diagnosis of clubfoot. Methods Twenty-four fetuses (27 fetuses) in suspected prehypertensive ultrasound during prenatal ultrasound screening from 20 to 26 weeks of gestation were enrolled in this study. A total of 27 normal fetuses from 20 to 26 weeks prenatal ultrasound screening were selected randomly 150 fetuses) as the normal control group. The angles between the plantar line and the long axis of the tibia (ie, the medial malleolus) of the first metatarsal metatarsal cross-section and the lateral metacarpal were measured respectively. The linear correlation between the two groups of data was analyzed. Results The normal fetuses had a constant measurement of the gestational week at the malleolus and had no significant correlation with gestational age (P> 0.05). The mean ± SD was (78.89 ° ± 9.24 °) and (37 ± 44) ± ° ± 7.93 °). Compared with the normal control group, the difference of the medial malleolus angle was statistically significant (P <0.01). Conclusion There is no significant correlation between the angle of normal malleolus and gestational age. The measured value can provide more accurate diagnostic information for prenatal diagnosis of clubfoot, which has a high clinical value.
