目的分析脑钠肽对儿童肺炎合并心力衰竭病情及预后的评估价值。方法选取2014年7月—2015年4月广西梧州市人民医院收治的肺炎合并心力衰竭患儿40例作为肺炎并心力衰竭组,同期无心力衰竭的肺炎患儿40例作为单纯肺炎组,体检健康儿童40例作为健康对照。比较3组儿童脑钠肽异常率和脑钠肽水平,并对肺炎并心力衰竭组不同心功能分级患儿脑钠肽水平进行分析。结果肺炎并心力衰竭组心力衰竭期患儿脑钠肽异常率高于健康对照组和恢复期,恢复期、单纯肺炎组患儿脑钠肽异常率高于健康对照组(P<0.05)。肺炎并心力衰竭组心力衰竭期患儿脑钠肽水平高于健康对照组、单纯肺炎组和恢复期,恢复期患儿脑钠肽水平高于健康对照组和单纯肺炎组,单纯肺炎组患儿脑钠肽水平高于健康对照组(P<0.05)。肺炎并心力衰竭组心功能Ⅲ级患儿脑钠肽水平高于Ⅰ、Ⅱ级患儿,Ⅱ级患儿脑钠肽水平高于Ⅰ级患儿(P<0.05)。结论脑钠肽对儿童肺炎合并心力衰竭病情及预后均有一定的评估价值,可反映患儿心力衰竭衰严重程度,可作为诊断儿童肺炎合并心力衰竭的重要诊断指标。
Objective To evaluate the value of brain natriuretic peptide in the diagnosis and prognosis of children with pneumonia complicated with heart failure. Methods From July 2014 to April 2015, 40 children with pneumonia complicated with heart failure admitted to Wuzhou People’s Hospital of Guangxi Province were selected as pneumonia and heart failure patients. 40 children with pneumonia without heart failure during the same period were enrolled in this study. 40 children as a healthy control. The abnormalities of brain natriuretic peptide and brain natriuretic peptide were compared between the three groups. The levels of brain natriuretic peptide in children with different cardiac function in pneumonia and heart failure group were also analyzed. Results The abnormal rate of brain natriuretic peptide in children with pneumonia and heart failure was significantly higher than that in healthy controls and convalescent and recovery periods. The abnormal rate of brain natriuretic peptide in children with pneumonia and heart failure was significantly higher than that in healthy controls (P <0.05). The levels of brain natriuretic peptide in patients with pneumonia and heart failure were significantly higher than those in healthy controls. The levels of brain natriuretic peptide in children with convalescent pneumonia and convalescence were higher than those in healthy controls and patients with pneumonia alone Brain natriuretic peptide was higher than healthy control group (P <0.05). Brain natriuretic peptide levels in patients with grade Ⅲ pneumonitis and heart failure were higher than those in grade Ⅰ and Ⅱ, and children with grade Ⅱ were higher than those in children with grade Ⅰ (P <0.05). Conclusion Brain natriuretic peptide has a certain value in assessing the prognosis and prognosis of children with pneumonia complicated with heart failure. It can reflect the serious degree of heart failure in children and can be used as an important diagnostic index for diagnosing childhood pneumonia complicated with heart failure.
