目的探讨高度近视合并白内障的小切口白内障摘出及人工晶状体植入术的效果。方法对35例(62眼)高度近视合并白内障行小切口白内障摘出及人工晶状体植入。眼轴长26~32.50mm,平均27.86mm,其中眼轴长≤30mm者48眼,眼轴长>30mm者14眼,观察术后视力及术中、术后并发症。结果眼轴长≤30mm的48眼中术后视力≥0.3者40眼(83.33%),眼轴长>30mm的14眼中术后视力≥0.3者6眼(42.86%),术中悬韧带部分断离2眼,后囊破裂2眼,但均植入人工晶状体。结论小切口白内障摘出及人工晶状体植入术,适合于高度近视合并白内障,但术后视力的恢复与眼轴长度、眼底黄斑功能密切相关,眼轴长>30mm者术后视力恢复较差。
Objective To investigate the effect of small incision cataract extraction and intraocular lens implantation in high myopia with cataract. Methods Small incision cataract extraction and intraocular lens implantation were performed in 35 cases (62 eyes) with high myopia and cataract. The axial lengths ranged from 26 to 32.50 mm with an average of 27.86 mm, including 48 eyes with axial length less than 30 mm and 14 eyes with axial length> 30 mm. Visual acuity and intraoperative and postoperative complications were observed. Results In 48 eyes with axial length ≤30mm, postoperative visual acuity ≥0.3, 40 eyes (83.33%) and axial length> 30mm, 6 eyes (42.86%) had postoperative visual acuity ≥0.3, 2 eyes, 2 posterior capsule rupture, but were implanted intraocular lens. Conclusions Small incision cataract extraction and intraocular lens implantation are suitable for high myopia with cataract. However, the recovery of visual acuity is closely related to axial length and macular function. The recovery of visual acuity after the axial length> 30mm is poor.
