“半蒌贝蔹芨攻乌”属中药“十八反”范畴,历来一直被视为配伍用药禁忌。延至现在,《中国药典》1985、1990年版仍规定:川乌(包括制川乌、草乌、制草乌)不宜与半夏、瓜蒌、贝母、白蔹、白芨、天花粉、犀角同用。同时还规定:附子不宜与半夏、瓜蒌、贝母、白笈同用。但历代医家争议颇多,相反药物同用者屡见不鲜,近代亦有提出不同意见的。
