目的:观察心包腔注射米力农对慢性心力衰竭大鼠模型血流动力学参数的影响,并探讨米力农心包腔给药与静脉给药的差异。方法:腹主动脉缩窄法制备大鼠慢性心力衰竭模型,38只成模大鼠随机分为心包组(n=19)和静脉组(n=19)。2组均给予0.5 mL米力农(50μg.kg-1),分别经心包腔和股静脉给药,监测给药前后大鼠平均动脉压(MAP)、心率(HR)、左心室收缩压峰值(LVSP)、左心室内压最大上升速率(LV+dp/dt)、左心室内压最大下降速率(LV-dp/dt)、左心室舒张期末压(LVEDP)的变化。结果:心包腔给药后5,15,30,60 min,LVSP、LV+dp/dt、LV-dp/dt较给药前显著增加(P<0.05),LVEDP显著降低(P<0.05),HR和MAP未见显著改变;静脉给药后1,5,15 min,LVSP、LV+dp/dt较给药前极显著增加(P<0.01),LV-dp/dt、HR显著增加(P<0.05),MAP、LVEDP显著降低(P<0.05)。给药后1,5,15 min,心包组MAP高于静脉组(P<0.05),而LVSP、LV+dp/dt、HR低于静脉组(P<0.05);30、60 min,LVSP、LV+dp/dt、LV-dp/dt高于静脉组(P<0.05)。结论:心包腔注射米力农可以提高心衰大鼠左心室的收缩功能和舒张功能,与静脉注射比较作用缓和、平稳、持久,并且对体循环血压和心率无影响。
Objective: To observe the effect of perioperative injection of milrinone on the hemodynamic parameters in rats with chronic heart failure, and to explore the difference between the intramyocardial administration and intravenous administration of milrinone. Methods: Chronic heart failure model was established by abdominal aorta constriction. Thirty-eight adult rats were randomly divided into pericardial group (n = 19) and intravenous group (n = 19). 0.5 mL milrinone (50 μg · kg-1) was administered to the rats in both groups, and the rats were administered pericardially and femoral vein separately. The mean arterial pressure (MAP), heart rate (HR) and the peak value of left ventricular systolic pressure LVSP, LV + dp / dt, LV-dp / dt and LVEDP were measured. Results: LVSP, LV + dp / dt and LV-dp / dt significantly increased (P <0.05) and LVEDP decreased significantly at 5, 15, 30 and 60 min after pericardial administration (P < HR and MAP did not change significantly. LVSP and LV + dp / dt increased significantly (P <0.01) and LV-dp / dt and HR increased significantly at 1, <0.05), MAP and LVEDP decreased significantly (P <0.05). At 1, 5, and 15 min after administration, MAP in pericardial group was higher than that in intravenous group (P <0.05), and LVSP, LV + dp / dt and HR were lower than those in intravenous group LV + dp / dt and LV-dp / dt were higher than those in the vein group (P <0.05). Conclusion: Pericardial injection of milrinone can improve the left ventricular systolic function and diastolic function in heart failure rats. Compared with intravenous injection, milrinone can relieve, smooth and lasting effect, and has no effect on systemic blood pressure and heart rate.
