目的探讨非ST段抬高型急性冠状动脉综合征(NSTE-ACS)患者的血清降钙素原(PCT)水平与冠状动脉病变程度的关系。方法选择经冠状动脉造影诊断为NSTE-ACS患者168例为研究对象,分为不稳定型心绞痛(UA)组和非ST段抬高型心肌梗死(NSTEMI)组,纳入89例经冠状动脉造影排除冠心病的患者为对照组。对NSTE-ACS患者冠状动脉病变狭窄程度进行SYNTAX评分,分为高、中、低分组。比较各组患者基本资料和血清PCT水平。结果 NSTEMI组、UA组患2型糖尿病、高血压和吸烟比例均高于对照组(P<0.05),但NSTEMI组和UA组间差异均无统计学意义(P>0.05);NSTEMI组、UA组血清PCT水平高于对照组,NSTEMI组高于UA组(均P<0.05)。低分、中分和高分组患高血压、高脂血症、吸烟比例和BMI比较,差异均无统计学意义(P>0.05);高分组和中分组2型糖尿病患者比例均高于低分组(P<0.05),但中分组和高分组间差异无统计学意义(P>0.05);高分组、中分组血清PCT水平高于低分组,高分组高于中分组(均P<0.05)结论血清PCT水平与冠状动脉严重程度相关,血清PCT水平对NSTE-ACS的严重程度和预后判断有重要参考意义。
Objective To investigate the relationship between the serum procalcitonin (PCT) level and the degree of coronary artery lesion in patients with non-ST segment elevation acute coronary syndrome (NSTE-ACS). Methods A total of 168 patients with NSTE-ACS diagnosed by coronary angiography were enrolled and divided into unstable angina pectoris (UA) group and non-ST-segment elevation myocardial infarction (NSTEMI) group. 89 patients underwent coronary angiography Coronary heart disease patients as control group. SYNTAX scores of coronary artery stenosis in patients with NSTE-ACS were divided into high, medium and low grouping. The basic information of each group and serum PCT levels were compared. Results There was no significant difference between NSTEMI group and UA group (P> 0.05). The incidence of type 2 diabetes mellitus, hypertension and smoking in NSTEMI group and UA group were higher than those in control group (P <0.05) The level of serum PCT was higher than that of the control group, and higher in the NSTEMI group than in the UA group (all P <0.05). The scores of hypertension, hyperlipidemia, smoking ratio and BMI in low, middle and high scores group had no significant difference (P> 0.05). The proportion of patients with high diabetes mellitus and moderate diabetes mellitus type 2 were higher than those in low scores (P <0.05), but there was no significant difference between the middle and high scores (P> 0.05). The serum PCT levels in the high and middle scores were higher than those in the low scores and high scores (P <0.05) The level of serum PCT correlates with the severity of coronary artery. The serum PCT level has important reference value for the severity and prognosis of NSTE-ACS.
