目的分析T_2DM患者使用人胰岛素治疗时不同的注射与进餐之间时间间隔(IMI)对血糖控制情况的影响。方法纳入92例T_2DM患者,分为两组,AB组为先接受有IMI的胰岛素治疗组,BA组为后接受有IMI的治疗组。主要观察指标为Hb A1c、血糖值、低血糖发生率及患者IMI方案偏好。结果取消IMI后,Hb A1c仅轻微增高,由于该差异没有临床相关性,因此无IMI的胰岛素疗法并不劣效于IMI的疗法(P<0.001)。取消IMI后患者的低血糖发生率也有所减少,但差异无统计学意义(P>0.05)。同时,两种疗法的血糖值也无统计学差异(P>0.05)。87.2%的患者倾向于无IMI的胰岛素注射法(P<0.001)。结论 2型糖尿病患者取消人胰岛素注射与进餐时间间隔对血糖总体控制情况无显著影响。
Objective To analyze the effect of different time intervals between injections and meals (IMI) on glycemic control in T 2DM patients treated with human insulin. Methods Ninety-two patients with T 2DM were enrolled and divided into two groups: group AB received insulin treatment with IMI first, and group BA received treatment with IMI after treatment. MAIN OUTCOME MEASURES: Hb A1c, blood glucose level, incidence of hypoglycemia, and patient IMI regimen preferences. Results After IMI was abolished, Hb A1c was only slightly increased, and insulin therapy without IMI was not inferior to IMI therapy (P <0.001), as the difference was not clinically relevant. The incidence of hypoglycemia in patients with IMI was also reduced, but the difference was not statistically significant (P> 0.05). At the same time, there was no significant difference in blood glucose between the two treatments (P> 0.05). 87.2% of patients preferred insulin injections without IMI (P <0.001). Conclusions The cancellation of human insulin injections and mealtime intervals in type 2 diabetic patients did not significantly affect overall glycemic control.
