目的 研究乌司他丁在兔急性肺损伤时对炎性细胞因子及氧自由基代谢的影响。方法 将 18只健康新西兰大白兔随机分为 2组。油酸组 (n =9)静脉注射分析纯油酸 0 12ml/kg ,复制急性肺损伤模型。乌司他丁组 (n =9)静脉注射乌司他丁5 0 0 0 0u/kg后立即静脉注射油酸 0 12ml/kg ,然后 ,再予乌司他丁 5 0 0 0u·kg-1·h-1持续静脉输注。两组皆在操作完成稳定 10min后 (基础值 )及注射油酸完毕后 1h、2h、3h、4h抽静脉血测血浆IL -8、MDA和活性氧含量 ,抽取动脉血作血气分析。结果 两组的IL -8、MDA和活性氧含量以及PaCO2 在注射油酸后逐渐上升 ,PaO2 逐渐下降 ,油酸组各观察指标的值的变化大于乌司他丁组 ,差异具有显著性。结论 乌司他丁能减少急性肺损伤中的IL -8及氧自由基的生成和释放。
Objective To investigate the effects of ulinastatin on the metabolism of inflammatory cytokines and oxygen free radicals in rabbits with acute lung injury. Methods Eighteen healthy New Zealand white rabbits were randomly divided into two groups. Oleic acid group (n = 9) intravenous injection of pure oleic acid 0 12ml / kg, replicate acute lung injury model. The ulinastatin group (n = 9) received intravenous injection of ulinastatin 5 0 0 0 0u / kg immediately after intravenous injection of oleic acid 0 12ml / kg, and then ulinastatin 5 0 0u · kg-1 H-1 Continuous intravenous infusion. Plasma levels of IL-8, MDA and reactive oxygen species in the venous blood were measured by venous blood at 1h, 2h, 3h and 4h after the operation was stabilized for 10min (basal value) and 1h, 2h, 3h and 4h after injection of oleic acid respectively. Results The levels of IL-8, MDA and reactive oxygen species and PaCO2 in two groups increased gradually after the injection of oleic acid, PaO2 gradually decreased, and the change of each observation index in oleic acid group was greater than that in ulinastatin group, the difference was significant. Conclusion Ulinastatin can reduce the production and release of IL-8 and oxygen free radicals in acute lung injury.
