目的探讨妊娠期糖尿病(GDM)患者胎儿脐动脉血流S/D值与剖宫产的关系。方法选择2014年1月-2015年12月海宁市妇幼保健院收治的120例GDM患者作为GDM组,120例正常孕妇作为对照组。测量两组孕妇的脐动脉收缩期最大血流峰值/舒张期最小血流峰(S/D)值。结果 GDM组患者脐动脉血流S/D值为2.93±0.92,对照组为2.46±0.73,两组比较,差异有统计学意义(P<0.05)。GDM组患者孕37周和孕38周脐动脉血流S/D值和对照组比较,差异无统计学意义(P>0.05),GDM组孕妇孕39周、孕40周、孕41周脐动脉血流S/D值高于对照组,差异有统计学意义(P<0.05)。GDM组胎儿窘迫的发生率高于对照组,差异有统计学意义(P<0.05)。GDM组剖宫产率高于对照组(P<0.05)。GDM患者脐动脉血流S/D值与胎儿窘迫、剖宫产均呈正相关(均P=0.000)。结论 GDM患者脐动脉血流S/D值、胎儿窘迫发生率和剖宫产率均升高,脐动脉血流S/D值与胎儿窘迫、剖宫产均呈正相关。
Objective To investigate the relationship between S / D of umbilical artery blood flow and cesarean section in gestational diabetes mellitus (GDM). Methods 120 cases of GDM patients admitted from January 2014 to December 2015 in Haining MCH were selected as GDM group and 120 normal pregnant women as control group. The maximal / diastolic minimum blood flow (S / D) values of systolic umbilical artery were measured in two groups of pregnant women. Results The S / D of umbilical artery in GDM group was 2.93 ± 0.92, while in control group was 2.46 ± 0.73, the difference was statistically significant (P <0.05). The S / D values of umbilical artery blood flow in the GDM group at 37 and 38 weeks of gestation were not significantly different from those in the control group (P> 0.05). The pregnant women in the GDM group had 39 weeks pregnant, 40 weeks pregnant and 41 weeks pregnant umbilical artery Blood flow S / D value was higher than the control group, the difference was statistically significant (P <0.05). The incidence of fetal distress in GDM group was higher than that in control group, the difference was statistically significant (P <0.05). Cesarean section rate in GDM group was higher than that in control group (P <0.05). The S / D values of umbilical artery blood flow in GDM patients were positively correlated with fetal distress and cesarean section (all P = 0.000). Conclusion The S / D value of umbilical artery blood flow, fetal distress rate and cesarean section rate in GDM patients were all increased. The S / D values of umbilical artery blood flow were positively correlated with fetal distress and cesarean section.
