目的确定吸烟对甲状腺相关眼病(TAO)患者眶脂肪分化的影响。设计回顾性病例系列。研究对象2012年6-12月在上海长征医院眼科行眶减压术的TAO患者40例,男性27例,女性13例。方法 40例TAO患者中,21例吸烟、8例曾吸烟但已戒烟,5例被动吸烟,6例非吸烟。运用RT-PCR检测所有患者眶脂肪CCAAT/增强结合蛋白(C/EBP)和过氧化物酶体增生物激活受体(PPARγ)mRNA表达量。用△Ct法进行其各基因表达的相对定量。多组间的总体比较采用单因素方差分析,随后用SNK-q检验进行两两比较。主要指标C/EBP和PPARγmRNA表达量。结果吸烟组患者的C/EBP(12.04±4.26)和PPARγmRNA表达量(4.82±1.41)明显高于非吸烟组(C/EBP:2.32±0.92;PPARγ:1.37±0.25)(P<0.001)。同时,曾吸烟组和被动吸烟组的C/EBP(4.29±1.26,5.76±1.03)和PPARγmRNA表达量(2.91±1.23,3.22±0.50)对比非吸烟组的差异也具有统计学意义(曾吸烟组C/EBP P=0.039,PPARγP=0.047;被动吸烟组C/EBP P=0.002,PPARγP=0.002)。吸烟指数≥100年支的TAO患者眶脂肪C/EBP(14.28±3.61)和PPARγmRNA(5.65±1.02)高于吸烟指数<100年支的患者(C/EBP:8.42±2.30;PPARγ:3.48±0.74)(P均<0.01)。结论眶脂肪C/EBP和PPARγmRNA的表达量与吸烟史有关。
Objective To determine the effects of smoking on orbital fat differentiation in patients with thyroid-associated ophthalmopathy (TAO). Design retrospective case series. Participants 40 patients with TAO undergoing orbital decompression in Shanghai Changzheng Hospital from June to December 2012, 27 males and 13 females. Methods Among 40 TAO patients, 21 were smokers, 8 smoked but smoked cigarettes, 5 were passive smokes, and 6 were non-smokers. The mRNA expression of orbital fat CCAAT / Cbp and PPARγ in all patients was detected by RT-PCR. The relative quantification of each gene expression was performed by △ Ct method. The overall comparison among groups was performed by one-way ANOVA followed by SNK-q test for pairwise comparisons. The main indicators of C / EBP and PPARγ mRNA expression. Results The levels of C / EBP (12.04 ± 4.26) and PPARγmRNA in smoking group were significantly higher than those in non-smoking group (C / EBP: 2.32 ± 0.92; PPARγ: 1.37 ± 0.25) (P <0.001). At the same time, the differences of C / EBP (4.29 ± 1.26, 5.76 ± 1.03) and PPARγmRNA (2.91 ± 1.23, 3.22 ± 0.50) between the smoker group and passive smoking group were also statistically significant C / EBP P = 0.039, PPARγP = 0.047; passive smoking C / EBP P = 0.002, PPARγP = 0.002). The orbital fat C / EBP (14.28 ± 3.61) and PPARγ mRNA (5.65 ± 1.02) were higher in patients with TAO than 100 years with smoking index ≥100 years (C / EBP: 8.42 ± 2.30; PPARγ: 3.48 ± 0.74 ) (P <0.01). Conclusions The expressions of C / EBP and PPARγmRNA in orbital fat are related to smoking history.
