用CA - 50IRMA ,β2 -MGRIA放射免疫法对 3 0名健康人 ,64例腮腺肿瘤患者的腮腺液、全唾液及血清中CA - 50和 β2 -MG含量进行分析 .结果 :3 0名健康者上述两项指标各类标本全都为阴性 .恶性肿瘤组CA - 50最灵敏 ,全唾液阳性率为 96 3 % ( 56 7± 2 3 2 )U/mL ,与健康、良性、非肿瘤组比较P值均<0 0 1;患侧腮腺液阳性率为 92 6% ( 4 4 0 3± 2 3 3 )U/mL ,健侧阳性率为 4 0 7% ( 2 3 82± 2 3 4 )U/mL ,健、患两侧分别与健康、良性、非肿瘤组比较 ,P值均 <0 0 1;血清阳性率仅为 7 4 % ( 13 9±2 2 8)U/mL ,与健康组比较P <0 0 5.良性肿瘤组CA - 50含量全唾液、患侧及健侧腮腺液、血清 4项标本阳性率分别为 9 0 % ,0 0 % ,4 5% ,9% .非肿瘤组CA - 50含量全唾液、健侧腮腺液均为阴性 ,患侧阳性率为 2 0 % .β2 -MG指标各组阳性率普遍较低 ,组间差异大多无显著性 ,恶性肿瘤组全唾液、健侧腮腺液及血清阳性率为 11 1%～ 11 4 % ,患侧腮腺液阳性率为 14 8% .良性肿瘤组及非肿瘤组各类标本 β2 -MG阳性率均在 0 %～ 2 0 %之间 .结论 :腮腺局部肿瘤放免诊断指标CA - 50优于 β2 -MG ,全唾液、腮腺液优于血清 ,CA - 50检测对腮腺恶性肿瘤诊断有较高的参考价值
The levels of CA - 50 and β2 - MG in parotid gland, saliva and serum in 30 healthy volunteers and 64 parotid gland tumor patients were analyzed by CA - 50IRMA and β2 - MGRIA radioimmunoassay.Results: The most sensitive and all saliva positive rate of CA - 50 in malignant group was 96 3% (56 7 ± 2 3 2) U / mL, which was significantly lower than that in healthy, benign and non - The positive rate of parotid gland in the affected side was 92 6% (4 4 0 3 ± 2 3 3) U / mL, and the positive rate of contralateral parotid gland was 4 0 7% (2 3 82 ± 2 3 4) U / mL, respectively. The P values were all <0 0 1 in healthy and benign and non-neoplastic groups, and the positive rate was only 74% (13 9 ± 2 28) U / mL in healthy and benign groups. The positive rates of CA - 50 in salivary, ipsilateral and contralateral parotid gland fluid and serum in benign tumor group were 90%, 0%, 45% and 9%, respectively The content of CA - 50 in whole saliva and contralateral parotid gland was negative, and the positive rate in affected side was 20%. The positive rates of β2-MG index in each group were generally lower than those in normal salivary gland , Contralateral parotid gland and serum positive rate of 11 The positive rate of parotid gland fluid in the affected side was 14.8%. The positive rates of β2-GM in all benign and non-neoplastic groups were between 0% and 20% .Conclusion: Local parotid radiotherapy The diagnostic index CA - 50 is superior to β2 - MG, and all saliva and parotid gland are superior to serum. The detection of CA - 50 has a higher reference value for the diagnosis of parotid malignancy
