目的 :观察萘替米星对老年肺炎的临床疗效 ,研究其在肺组织中的浓度。方法 :49例老年肺炎患者为对象 ,观察萘替米星对其疗效 ,并通过TDX荧光偏振免疫分析仪测定了 16例患者血清及痰液中的药物浓度 ,以酶偶联速率法测定尿素浓度 ,并计算肺组织中萘替米星稀释倍数。结果 :萘替米星对老年肺炎临床总有效率85 71% ,痊愈率 67 3 5 % ;痰细菌清除率 86 11% ,低浓度 (1ml/L)即对革兰氏阴性杆菌有抗菌活性 ,4~ 8mg/L可抑制 60 %~ 85 %的细菌生长。结论 :萘替米星每天全量给予 ,可获得较高的血药浓度 ,并在肺泡中达到高的药峰浓度 ,可通过测定血清药物浓度预测肺泡药物浓度
Objective: To observe the clinical efficacy of netilmicin on senile pneumonia and to study its concentration in lung tissue. Methods: 49 cases of elderly patients with pneumonia were studied, and the effect of netilmicin was observed. Low concentration (1 ml / L) had antibacterial activity against gram negative bacilli, 4 ~ 8mg / L can inhibit the growth of 60% to 85% of bacteria. CONCLUSIONS: Netilmicin is given daily in full dose to obtain higher plasma concentration and high peak concentration in the alveoli, and the concentration of alveolar drug can be predicted by measuring serum drug concentration
