目的:为评价射频导管消融在治疗儿童房室结折返性心动过速的临床价值,本文回顾分析37例儿童射频消融结果评价、安全性及疗效。方法:选择6~15岁房室结折返引起频发的、药物治疗无效的心动过速患儿,常规电生理检查,采用下位法消融慢径路。结果:37例中房室结双径路32例、三径路5例,全部消融成功,并发Ⅰ度AVB 1例,消融电极压迫出现一过性Ⅲ度AVB 3例,复发率13.5%,随访中无房室传导阻滞及X线放射反应。结论:射频消融术治疗儿童房室结折返性心动过速安全有效,可以在有丰富射频消融经验的大医院内开展。
OBJECTIVE: To evaluate the clinical value of radiofrequency catheter ablation in the treatment of children with atrioventricular nodal reentrant tachycardia, we retrospectively analyzed the evaluation, safety and efficacy of radiofrequency ablation in 37 children. Methods: Atrioventricular nodal reentry from 6 to 15 years old was used to induce frequent tachycardia in children with invalid tachyarrhythmia. Conventional electrophysiological examination was used to ablate the slow pathway. Results: Thirty-two cases of mid-atrioventricular node dual-path pathway in 32 cases and three-path pathway in all cases were successfully ablated. One case had grade Ⅰ AVB complicated by ablation electrode and 3 cases had transient AV Ⅲ degree Ⅲ. The recurrence rate was 13.5% Atrioventricular block and X-ray radiation. Conclusion: Radiofrequency catheter ablation is a safe and effective treatment of children with atrioventricular nodal reentrant tachycardia and can be carried out in a large hospital with rich experience in radiofrequency ablation.
