目的探讨血浆脂联素和胰岛素水平与儿童肥胖症发病的关系。方法选择2007年4月—2010年10月来南阳市中心医院初诊并住院诊治的肥胖儿童40例和健康儿童50例,设为肥胖组及对照组。测定两组的空腹血清脂联素、胆固醇、甘油三酯(TG)、低密度脂蛋白-胆固醇(LDL-C)、高密度脂蛋白-胆固醇(HDL-C)、血糖和胰岛素,并分析脂联素与体重指数(BMI)、血脂、血糖和胰岛素的相关性。结果①肥胖组血浆脂联素水平低于对照组(P<0.01);肥胖组的血糖和胰岛素水平显著高于对照组(P<0.01);②肥胖儿童的血脂各指标与对照组相比,LDL-C和TG明显高于对照组(P<0.05、P<0.01);③相关性分析:血浆脂联素与BMI、TG、空腹血糖、空腹胰岛素呈负相关(P<0.01)。结论脂联素水平的变化与儿童肥胖症的发病有关,是影响空腹血糖、空腹胰岛素水平的重要因素。
Objective To investigate the relationship between plasma adiponectin and insulin levels and the incidence of childhood obesity. Methods From April 2007 to October 2010, 40 obese children and 50 healthy children, newly diagnosed and hospitalized in Nanyang Central Hospital, were selected as the obesity group and the control group. Fasting serum adiponectin, cholesterol, triglyceride (TG), low density lipoprotein-cholesterol (HDL-C), high density lipoprotein cholesterol (HDL-C), blood glucose and insulin were measured in both groups. Related factors and body mass index (BMI), blood lipids, blood glucose and insulin correlation. Results ① The level of plasma adiponectin in obese group was lower than that in control group (P <0.01); the level of blood glucose and insulin in obese group was significantly higher than that in control group (P <0.01); ② Compared with control group, LDL-C and TG were significantly higher than those in the control group (P <0.05, P <0.01); ③ Correlation analysis: Plasma adiponectin was negatively correlated with BMI, TG, fasting blood glucose and fasting insulin (P <0.01). Conclusion The change of adiponectin level is related to the onset of childhood obesity and is an important factor affecting the fasting blood glucose and fasting insulin levels.
