目的 探讨应用犬自体带血管蒂的肋骨肌瓣环修复胸内长段环形缺损气管的可行性。方法 选用成年杂种犬 18只 ,将第三肋骨及上下肋间肌修整成带蒂的骨肌瓣环 ,以此来替代切除的气管环。结果 18只犬术后存活 11只 ,生存时间最长的犬达 14个月。这些犬尽管再造气管环轻度狭窄 ,但可以自由活动。组织学检查显示肋间肌组织血供良好 ,环的内层胸膜间皮被纤毛上皮替代。结论 肋骨肌瓣环替代胸内气管具有操作简便、并发症少的特点。本研究结果提示该术式有望应用于临床。
Objective To investigate the feasibility of repairing the trachea of long annular defect in the chest using the cannulated rib muscle flap of dogs. Methods Eighteen adult mongrel dogs were selected. The third rib and upper and lower intercostal muscles were trimmed into pedicled musculoskeletal rings to replace the excised tracheal rings. Results Eighteen dogs survived after surgery and the longest lived dogs reached 14 months. Although these dogs are mildly constricted in regenerating the tracheal rings, they are free to move. Histological examination showed good blood supply to intercostal muscle tissue, and the inner pleural mesothelium of the ring was replaced by cilia epithelium. Conclusions The replacement of intrathoracic trachea with rib musculature ring has the advantages of simple operation and few complications. The results of this study suggest that the surgical technique is expected to be applied to the clinic.
