After some early trials, Achbaugh et al began breathing in 1967 in adults with positive end-exp-piratory pressure (PEEP). Fourteen years later, the law was quickly recognized and widely used. PEEP is primarily used to treat pulmonary edema (ie, adult respiratory distress syndrome, ARDS) or increased hydrostatic pressure (mainly cardiogenic pulmonary edema) with increased alveolar capillary membrane permeability. In contrast to natural respiration, PEEP has the potential to increase end-expiratory lung volume through airway opening, alveolar expansion, and possible redistribution of fluid in the lungs. In conclusion, these effects are due to reduced intrapulmonary blood
