目的近年来,二氟甲基鸟氨酸(difuloromethylornithine,DFMO)这一“老药”在肿瘤,尤其是实体瘤防治领域重新受到重视,本研究拟对目前国内外有关其防治肿瘤的研究进展作一综述。方法应用PubMed和CNKI期刊全文数据库检索系统,以“二氟甲基鸟氨酸、肿瘤、化学预防和化学治疗”为关键词,检索2000-01-2016-01有关文献。共检索英文文献175篇,中文文献16篇。纳入标准:(1)DFMO预防和治疗肿瘤的基础研究;(2)DFMO预防和治疗肿瘤的临床药物试验。排除标准:(1)DFMO制备技术的研究;(2)其他鸟氨酸脱羧酶(ornithine decarboxylase,ODC)抑制剂的研究。最终分析文献33篇。结果 DFMO防治肿瘤的机制主要包括抑制ODC从而降低体内多胺水平和影响肿瘤免疫微环境等。目前的动物及临床试验发现其单药或与其他药物联用,对多种实体恶性肿瘤,尤其是结肠癌等上皮来源的肿瘤有较好的防治效果。并且DFMO还具有其毒副作用低、价格相对低廉、从科研到临床转化迅速等多方面优点。结论作为一种有效、安全的化疗预防或治疗药物,DFMO单药或与其他治疗方式的联合应用体现出令人期待的前景。但为了达到“老药新用”的目的,大量研究工作仍有待进一步展开。
Objectives In recent years, difuloromethylornithine (DFMO), an “old drug”, has received great attention in the field of tumor prevention and treatment, especially solid tumors. This study is intended to review the current research on the prevention and treatment of tumors in China and abroad. A summary of progress. Methods PubMed and CNKI journal full-text database retrieval system was used to search for relevant literature of 2000-01-2016-01 with the key words of “difluoromethylornithine, tumor, chemoprevention and chemotherapy”. A total of 175 English literature and 16 Chinese literature were retrieved. Inclusion criteria: (1) Fundamentals of DFMO in the prevention and treatment of cancer; and (2) clinical drug trials of DFMO in the prevention and treatment of tumors. Exclusion criteria: (1) DFMO preparation techniques; (2) other ornithine decarboxylase (ODC) inhibitors. The final analysis of 33 articles. Results The mechanism of DFMO in the prevention and treatment of cancer mainly includes the inhibition of ODC to reduce the level of polyamines in the body and the impact of tumor immune microenvironment. Current animal and clinical trials found that its single drug or combination with other drugs on a variety of solid malignant tumors, especially colon cancer and other epithelial tumors have better control effect. And DFMO also has its toxic side effects, the price is relatively low, from scientific research to rapid clinical transformation and many other advantages. Conclusion As a potent and safe chemoprevention or treatment drug, the combination of DFMO alone or in combination with other therapies offers promising prospects. However, in order to achieve the purpose of “old medicine and new use,” a great deal of research remains to be carried out.
