目的探讨2型糖尿病患者翼螺旋转录因子叉头框A2(Foxa2)基因突变与胰岛素抵抗和胰岛β细胞功能的影响,分析其之间的关系。方法选取2型糖尿病患者70例为观察组,健康体检者50例为对照组,两组患者入选后给予75g葡萄糖耐量试验,观察两组患者胰岛素抵抗及胰岛β细胞功能情况,对观察组患者进行Foxa2基因检测,利用DNAStar 4.0软件将所有PCR扩增产物测序,并与Gen Bank提供的启动子和外显子序列进行比对分析,观察比较Foxa2基因突变者与未突变者胰岛β细胞功能差异情况。结果观察组患者Homa-IR为5.81±3.59,高于对照组的2.61±1.28,差异有统计学意义(P<0.05)。IAI、Homa-β、△I30/△G30、MBCI、DI分别为0.01±0.01、101.48±84.15、4.03±3.25、8.54±4.16和21.13±11.35,均低于对照组,差异均有统计学意义(P<0.05)。观察组患者经Foxa2基因检测见突变者总计55例(78.6%),Foxa2基因存在突变者Homa-IR高于未突变者,而IAI、Homa-β、MBCI低于对照组,差异有统计学意义(P<0.05)。结论2型糖尿病患者Foxa2基因突变与其胰岛素抵抗和胰岛β细胞功能减退有关。
Objective To investigate the effect of Foxa2 gene mutation on insulin resistance and islet β-cell function in patients with type 2 diabetes mellitus (T2DM) and to analyze its relationship. Methods 70 cases of type 2 diabetes mellitus were selected as the observation group and 50 cases of healthy subjects as the control group. The two groups were given 75g glucose tolerance test after being enrolled. Insulin resistance and pancreatic β-cell function were observed in both groups. Foxa2 gene detection, using DNAStar 4.0 software all PCR amplification products were sequenced and GenBank with the promoter and exon sequences were compared to analyze the comparison Foxa2 gene mutation and non-mutated pancreatic β-cell function differences . Results The Homa-IR of the observation group was 5.81 ± 3.59, which was higher than that of the control group (2.61 ± 1.28, P <0.05). IAI, Homa-β, △ I30 / △ G30, MBCI, DI were 0.01 ± 0.01, 101.48 ± 84.15, 4.03 ± 3.25, 8.54 ± 4.16 and 21.13 ± 11.35, respectively, which were lower than the control group P <0.05). In observation group, a total of 55 cases (78.6%) were found mutation of Foxa2 gene. Homa-IR of Foxa2 gene mutation was higher than that of non-mutation, IAI, Homa-β and MBCI were lower than the control group, the difference was statistically significant (P <0.05). Conclusion The Foxa2 gene mutation in patients with type 2 diabetes mellitus is associated with insulin resistance and impaired pancreatic β-cell function.
